Objective:To understand the current situation of occupational stress and health of coal miners in Xinjiang, and to provide scientific basis for effective preventive measures and strategies for coal workers' physical and mental health. Methods: A cluster sampling method was used to investigate 1400 coal miners who had been working for more than one year in Xinjiang.Participants were give self-reported questionnaires including general demographic data,Effort-Reward Imbalance(ERI),Maslach Burnout Inventory(MBI)and Self-Rating Depression Scale(SDS),while randomly selected 508 coal miners for occupational health examination. Results: 1.A total of 1400 questionnaires were issued and 1334 questionnaires were collected. The effective rate was about 95.28%.2.Different gender and shift of coal miners in ERI each module scores were different(P<0.05),male scored higher than female, two shifts scored higher than other groups;various nationalities in work pay and input scored differently(P<0.05);the highest scores of ?30 years of service in work pay, with the increase of working years, the higher scores of job reward;transport operators the lowest score in work pay, while safety supervision workers the highest scores in job reward;different years of income were only different in work pay scores(P<0.05);there were differences in ERI of coal miners with different nationalities, working years, work type and education.3.The detection rate of occupational burnout was51.1%.Male's job burnout was higher than that of female;Han's emotional exhaustion score was higher than that of Minority Nationalities;with the increase of the length of service, the scores of emotional exhaustion, depersonalization and burnout were also increasing;coal miners whose worked four shifts with three changing had the highest scores of emotional exhaustion, depersonalization and burnout in the three groups;different degrees of education in burnout scores were diverse(P<0.05);different years of income were only different in personal accomplishment scores(P<0.05);safetysupervision workers the highest scores in exhaustion, depersonalization and burnout;with the increase of occupational stress level, the level of job burnout was also increasing.4.The incidence of depressive symptoms in coal miners was 35.5%.The incidence of depressive symptoms was different with different sex, type of work and shift(P<0.05).male was higher than female;the incidence of depressive symptoms was highest in coal miners whose were safety supervision workers and two shifts;the higher the level of occupational stress and job burnout, the higher the detection rate of depressive symptoms.5.Job burnout was positively correlated with occupational stress.Depression was positively correlated with work pay and input, emotional exhaustion and depersonalization, and negatively correlated with job reward and personal accomplishment.6.Gender,working time, work pay and ERI were the main factors influencing coal workers' occupational burnout;working time, shift, work pay, work reward,ERI,emotional exhaustion, depersonalization and occupational burnout level were the main factors affecting the coal mine workers' depressive symptoms(P<0.05).7.The top five of the abnormal detection rate of physical health were hyperlipidemia, hypertension,ECG abnormality, liver dysfunction and fatty liver. There were differences in the detection rates of hyperlipidemia among coal miners with different working time, type of work, marital status, annual income, alcohol consumption and body mass index(P<0.05);the detection rate of hypertension in different working years, work type, shift, marital status, alcohol consumption and body mass index were different(P<0.05);different degrees of education and body mass index of coal workers' ECG abnormal rate were distinct(P<0.05);different sex, alcohol consumption and body mass index of coal workers' liver dysfunction rate were diverse(P<0.05);the detection rate of fatty liver were distinct in various working years, shifts, annual income, alcohol consumption and body mass index(P<0.05);with the increase of the length of service, the detection rate of hyperlipidemia, hypertension and fatty liver were also increasing;often drinking and obese people with hyperlipidemia, hypertension, liver dysfunction and fatty liver detection rate were highest;with the increase of the level of job burnout, the detection rate of hyperlipidemia was increasing;with the increase of the level of depression, the detection rate of hypertension was also increasing, while the detection rate of hyperlipidemia was decreasing.8.Age,income,BMI were risk factors of hyperlipidemia;age, shift, alcohol consumption,BMI,ERI and the level of depression were risk factors for hypertension;working time and BMI were risk factors of ECG abnormality;shift, smoking, drinking, annual income and BMI were risk factors for fatty liver. Conclusion: The coal miners occupational stress condition wasserious;the higher degree of occupational stress and mental health state of the worse;cardiovascular disease and liver disease were the focus of prevention and treatment.
