ObjectiveAs we all know, mixed hemorrhoid is a common disease, and the surgery is one of the best solutions for the treatment of moderate and severe mixed hemorrhoids. However, dysuria often happens after the operation of mixed hemorrhoids, and it can greatly affect the patients' medical experience. It has been confirmed that tamsulosin and acupuncture had a good curative effect on postoperative dysuria in current literatures. However, the treatments were only used when dysuria had already happened, and there' s little attention being paid to the prevention of this complication. So,in this study,we explored the etiology and pathogenesis of dysuria after hemorrhoid operation in order to find a way to prevent the dysuria effectively, and we believed that this can greatly improve the patients' medical experience and life quality after operation. It can also provide an innovative prevention program for clinicians by using the scientific method to evaluate the safety and effectiveness of the treatments combining tamsulosin with acupuncture.MethodsThis study was a prospective randomized controlled clinical trial. We collected 150 hospitalized patients meeting the inclusion criteria from October 2016 to February 2017 in Guangdong Province Hospital of traditional Chinese Medicine. These patients were randomly and equally divided into three groups:group of combination with acupuncture and drugs, group of the single use of Harnal, and the control group.Patients in combined usage group were treated with oral Harnal 0.2mg four hours after surgery, when they had micturition desire, they would be needled zhongji, guanyuan, qihai, bilateral sanyinjiao and bilateral taichong as acupunture points. These points must be retained needling for 15 minutes after "De Qi".The patients in single usage group were only be treated with oral Harnal 0.2mg four hours after surgery, and there were not any acupuncture.Patients in control group were not advanced interfered.Observation indice included the postoperative dysuria, the first postoperative voiding time, urine volume for the first time, whether postoperative catheterization and total volume of urine in eight hours after operation. We biult all the datum into a database by SPSS19.0.All the data imformations were input twice in order to prevent the entry error. In the aspect of statistics, comparisons between groups were tested by ANOVA,Chi-square test and rank test. The test level alpha is 0.05.ResultsThe baseline indice such as gender, age and course of disease were balanced distributed in these three groups. It was well comparable between the groups as there were no statistics differences in baseline indice. After tested by statistical methods,we found that some of the observation indice had statistics differences, which included the postoperative dysuria, the first postoperative voiding time, urine volume for the first time, total volume of urine in eight hours after operation and the urinary flow rate for the first time after surgery.ConclusionThe research results show that both the combined usage and the single usage have curative effects on the prevention of dysuria after operation of mixed hemorrhoids.Being compared with the treatmengt of single use,the treatment of combination use has more obvious advantages in the aspects of postoperative voiding for the first time, urine volume for the first time and the urinary flow rate for the first time after surgery. This means that the combined usage is more effective and safer in the prevention of postoperative dysuria. And we think it is deserves to be spread in clinic. For the patients who do not accept the treatment of acupuncture, they can also be treated with Harnal only to prevent the postoperative dysuria. Thus,we can improve the patients' medical experience.
