Assessing Life Quality For Patients With Hypertension In Lianyungang Rural Regions Based On EQ-5D

Objective:By using the EQ-5D,We investigate the quality of life in physiological and psychological dimensions of hypertension in Lianyungang rural areas.To find the factors that affect the quality of life,then we can provide scientific basis for the health management of hypertensive patients in the rural area of Lianyungang.Methods : From July to August,in the five towns of Lianyungang(Shiliu streets,Hongzhuang town,Qinghu town,Wenquan town and Zhangwan township),we selected2125 hypertensive patients as the study subjects,organized them to the local hospitals for blood pressure measurement,physical examination and questionnaires,attained their fundamental information,evaluated the quality of life by the EQ-5D Scale.We used Epidata3.1 to establish a database,the original data was entered for twice,to ensure correctness,used SPSS16.0 software to analysis the data.Results:(1)The average systolic pressure and diastolic pressure are(146.66±12.69)mm Hg and(94.37±12.01)mm Hg.The medication rate of hypertensive patients was 63.0%,blood pressure control rate of hypertensive patients was 13.1%.(2)The five dimensions of EQ-5D scale: 327(15.3%)peoples with difficulty in mobility;103(4.8%)peoples with difficulty in self-care;211(9.9%)peoples with difficulty in usual activities;960(45.1%)peoples with difficulty in pain / discomfort;358(16.9%)peoples with difficulty in anxiety / depression.By using the UK TTO conversion table,the score of EQ-5D(0.839 ± 0.217),from-0.349 to 1,mobility(0.011 ±0.026),self-care(0.005 ± 0.025),usual activities(0.004 ± 0.014),pain / discomfort(0.066 ± 0.089),anxiety / depression(0.015 ± 0.041).(3)The difficult proportion of five dimensions in diverse towns with hypertension was different,the patients of Shiliu street was in the highest proportion except the dimension of mobility.The normal blood pressure group was in the higher proportion in the dimensions of mobility and usual activities.Simple systolic pressure has a higher proportion of problems in the dimension of mobility.(4)The protective factors of mobility problem are: education,monthly average income,risk factors are: age,history of hypertension;The protection factors of self-care problem are: gender,monthly average income,risk factors are: occupation;The protective factors of usual activities problem are: monthly average income,blood pressure grade,risk factors are: occupation,history of hypertension;The protective factors of pain /discomfort problem are: monthly average income,risk factors are: gender,marital status,The history of hypertension;The protective factors of anxiety / depression problems are:age,monthly average income,risk factors are: gender,education.(5)EQ-5D index is in a high score(0.839 ± 0.17),the average score of EQ-VAS is(70.06 ± 19.04),the patient's health status is good.The EQ-5D score was correlated with the EQ-VAS score.(6)The influencing factors of EQ-5D score were gender,monthly average income,marital status,history of hypertension,the influencing factors of EQ-VAS score were gender,BMI,monthly average income,history of hypertension,medication.Conclusions: The patients with hypertension in this survey area is in a high quality oflife,the quality of life of different towns' patients is different,the government still need to increase investment to protect the quality of life of the residents,the patients themselves should strengthen the management of hypertension,change unhealthy behavior,enhance the risk awareness,measure blood pressure in regular,medication on time,improve their own quality of life.
