An Analysis Of Status And Effect Factors Of Self-injection Of Insulin In Diabetic Foot Patients

Objective Investigation in patients with diabetic foot of insulin injection behaviors on his own.;To find out the weak link of injection technology,and to analyze the main influencing factors;The aim of this study was to provide reference for the related education and preventive measures for health care workers,to promote and standardize the behavior of insulin injection and improve the therapeutic effect of insulin.Methods 1.The researchers reviewed the literature and made questionnaires.After the pre experiment and expert consultation,the questionnaire of insulin injection behavior of diabetic foot patients was finally formed.<My views on insulin > scale is a mature scale for use at home and abroad,The form is divided into main tables and subsidiary tables.2.This research using convenience sampling,Choosing 236 patients with diabetic foot who met the inclusion criteria and self-injecting of insulin from September 2015 to June 2016 in a Diabetic foot treatment center in some third-grade class-A hospital of Dalian.3.After the consent of patients,the researchers asked patients to fill out natural information and clinical information questionnaires by consulting their medical records.the questionnaire of insulin injection behavior of diabetic foot patients and <My views on insulin > scale filled by patients.For patients who could not finish the questionnaire,we ask them to work out.4.Application of Epidate3.0 software for data entry,using SPSS 19 software for statistical analysis.The measurement data are expressed as the mean ± standard deviation((?)±s).Count data expressed as rate and percentage.Correlation analysis using Spearman correlation analysis.Single factor analysis of diabetic foot patients who inject insulin was performed using two independent sample t test and one-way anova,Multiple factors analysis using multiple stepwise regression analysis.Results 1.In 236 diabetic foot patients,the averagescore of the questionnaire is 44~110(79.41±13.387).The results of this study show that diabetic foot patients in the insulin injection technique aspect,that the scores of which is lowest(3.00±0.659).In this study,there are 5 items with lower scores,those are the weak link of standard injection.The score of use alcohol to disinfect the skin before injection is(2.92±0.871);The score of before refillable checked insulin liquid is(2.56±0.726);The score of how to dispose of an insulin needle is(1.91±0.639);The score of rotate between different parts is(1.67±0.156);The score of whether you regularly change the injection site is(1.58±0.099).Among them,there are 4 items belong to the insulin injection technology.2.In 236 diabetic foot patients,psychological insulin resistance was present in 88.6% of the patients.The total score of insulin injection in diabetic foot patients was positively correlated with the score of psychological insulin resistance(r=0.133,P < 0.05).3.Analysis of the characteristics in different population,the difference was statistically significant in diabetic foot patients in different sex with the behavior of self injection of insulin(P<0.05);There were statistically significant differences in different types of diabetic patients with diabetic foot self injection of insulin action(P<0.05);There were statistically significant differences in the number of daily injections of insulin in patients with diabetic foot of the different behavior of self injection of insulin(P<0.05);Whether the diabetic foot patients regularly monitor blood glucose by insulin injection behavior was statistically significant difference(P<0.05);There were statistically significant differences in diabetic foot patients according to whether diabetes diet eating the behavior of self injection of insulin(P<0.05);There were statistically significant differences in diabetic foot patients if follow doctor's advicewith the behavior of self injection of insulin(P< 0.05).4.Multiple stepwise regression analysis in patients with diabetic foot,gender,medical expenses,insulin injection time,patients with diabetic foot level,whether the regular monitoring of blood glucose,whether the diabetes diet eating,psychological insulin resistance score into the regression equation,are the main influencing factors of patients with diabetic foot self injection of insulin action.Conclusions 1.236 patients with diabetic foot in the insulin injection technology is the weakest.There is no normative behavior in the case of use alcohol to disinfect the skin of the injection site,check the insulin liquid before replacing the refill,how to deal with the discarded needles,the rotation of the injection site,whether replacement of injection site regularly.2.Patients with diabetic foot who use insulin have poor psychological condition.And the behavior of insulin injection was related to the level of psychological insulin resistance in diabetic foot patients 3.Gender,medical expenses,insulin injection time,patients with diabetic foot level,whether the regular monitoring of blood glucose,whether the diabetic diet,psychological insulin resistance are the main influencing factors of patients with diabetic foot self injection of insulin action.
