| Objective:Through the investigation and analysis of the leakage of the liquid after injection of insulin pen,the factors influencing the leakage of the liquid and the degree of its influence were studied.The intervention measures were carried out to reduce or avoid the leakage of the liquid,so that the insulin injected into the patients Dose consistent with the dose to ensure that the clinical efficacy;At the same time to explore a more rigorous scientific insulin pen injection procedures for clinical colleagues for reference.Methods:1.Retrospective nested case-control study: Collection of data using a self-made survey scale(see appendix),including: the basic situation of the sample population,disease-related status and insulin pen injection situation.The survey was conducted using the cluster sampling method.From June 1,2015 to December 31,2015,92 patients who were admitted to the Department of Endocrinology in the First Affiliated Hospital of Wannan Medical College were enrolled in the case of type 2 diabetes mellitus Normalized injection(3 injections per person),and observe whether the drug after the injection of leakage,the collection of patient-related clinical data,the impact of insulin injection after drug injection of potential factors for single factor and multivariate analysis,to explore the real Influencing factors,and analyzing their influence degree.2.Intervention study: According to established factors to intervene,the same use ofcluster sampling method,from January 1,2016 to October 30,2016,92 patients who were admitted to the Department of Endocrinology in the First Affiliated Hospital of Wannan Medical College were enrolled in the case of type 2 diabetes mellitus Normalized injection(3 injections per person),was performed.The infusion of 12 different doses of insulin(4u,6u,···,26U)was observed under the condition of 15 seconds after the injection of the needle,and the patient’s related clinical data were collected,and he positive rate of leakage occurred compared with the baseline survey.Results:1.Retrospective nested case-control study: The investigation of type 2 diabetes patients after injection of insulin pen injection phenomenon is more common,a total of92 patients eligible for a standardized insulin pen injection 276 times,observed after injection of 158 cases of leakage occurred,The incidence of leakage is about 57.2%;especially when the injection dose is large,the occurrence of a higher proportion.The results of univariate analysis showed that the subcutaneous dwell time(χ2= 8.984,P<0.05)was significant at the 0.05 level after the insulin pen injection.The logistic regression analysis showed that the injection dose of insulin pen(χ2= 17.874,P <0.05)and subcutaneous residence time(χ2= 21.581,P <0.05)were significant after insulin injection.And the ROC curve further analyzes the subcutaneous residence time and obtains a cut-off value,that is,when the subcutaneous residence time is 14.5 seconds,the incidence of leakage occurs.2.Intervention study: In this study,insulin was injected subcutaneously with seven doses of insulin for a total of 93 patients,with a standardized insulin pen injection of279 patients,the incidence of leakage was 20.1%,and before the intervention of the seven different units of insulin injection Of the leakage rate of 66.4% compared to the decline of 3 times.And when the subcutaneous residence time of 15 s,insulin subcutaneous dose of more than 20 u,the incidence of leakage close to 100%.Conclusion:Insulin pen injection tip in the subcutaneous residence time and injection dose is the impact of insulin injection after the injection of liquid influencing factors,which after the injection of the tip of the needle in the subcutaneous residence time is the insulin pen injection after the leakage of protective factors,insulin pen injection Subcutaneous stay at least 15s;dose is a risk factor,subcutaneous insulin dose is not easy to more than 20u;can effectively avoid or reduce the leakage of insulin after injection of liquid. |
