Objective:This study aimed to investigate the safety and clinical efficacy of water needle knife by ultrasound-guided technique on lumbar disc herniation through assessing changes of clinical symptoms, clinical signs and patients' daily life and work. And then to a safe, economic and effective therapeutic method that would significantly reduce symptoms and improce patient quality of life.Meanwhile,it provides a new thinking for the clinical use of needle knife on patients with lumbar disc herniation. Methods:A total of 60 patients with lumbar disc herniation from Guangdong Provincial hospital of Traditional Chinese Medicine during December of 2015 to December of 2016 were selected, which were randomly divided into ultrasound-guided group(U)and experience group(E),with 30 cases in each group.Patients in experience guided group were treated by doctor with water needle knife therapy under experience, while patients in ultrasound-guided group were treated with water needle knife under ultrasound guidance. The Visual Analogue Scale(VAS)?Oswestry disability index(ODI)evaluations and Japanese Orthopaedic Association(JOA)score were measured in all the patients before treatment and 1 week?1?3months after treatment, and adverse reactions were also recorded.VAS and ODI scores before and after treatment and curative effect were statistically analyzed. Results:There was no difference among the patient' age gender duration between two groups before treatment.Compared with before treatment, observed indicators were improved obviously in two groups after treatment, and the differences were statistically significant(P<0.05).Before treatment,VAS score,ODI evaluations and JOA score showed no significant difference between two groups(P>0.05),but the VAS score and ODI evaluation after treatment in ultrasound-guided group were significantly lower, and the JOA score inultrasound-guided group was higher than experience group(P<0.05).No obvious complications were found in all patients during follow-up.Conclusion:Ultrasound-guided water needle knife therapy can effectively improve the accuracy of injection and reduce adverse reactions, which is more effective for treatment of lumbar disc herniation and help to improve the curative effect. It is a safe, reliable and visualized therapeutic method, which is worthy of further clinical promotion.
