ObjectiveThis study was designed to investigate the current status of quality of life and hope level after operation,to analyze the relationship between quality of life and hope level of patients,to study the influence of general demographic information,disease characteristics and hope level on the quality of life in patients with bladder cancer and to provide the scientific theoretical basis for the intervention of clinical care staff.MethodsWe selected 118 inpatients with bladder cancer from Shandong Provincial Hospital by convenient sampling.The general data questionnaire,the life of quality scale for bladder cancer QLICP-BL(V2.0)and Herth Hope Scale were used for cross-sectional survey.The questionnaires were completed by the patients themselves,and used to assess the current status of quality of life and the hope level and to explore the main influencing factors of quality of life in bladder cancer patients.SPSS 17.0 software was used for statistical analysis,including descriptive analysis,independent t-test,single factor analysis of variance,Pearson correlation analysis,and multivariate linear regression analysis.Results1.The general demographic and disease information of patients with bladder cancerA total of 118 cases of bladder cancer patients were involved in the study,including 93 males and 25 females.The patient ages ranged from 27 to 86 years,with a mean age of 62.56± 13.04 year.Among the patients,teachers and cadres accounted for 21%,with workers 24.6%and farmers 42%.75.5%of the patients hadn’t received college education,with other 24.5%with a college degree or above.78%of the patients had the income of medium above and 95.8%of the patients had medical insurance.79.7%of the patients had a history of smoking,and 17.8%had quit.The patients with balanced diet accounted for 69.5%and the patients exercised regularly accounted for 44.9%.All the patients received surgical therapy,with 83.9%with transurethral resection of bladder tumor and 10.2%with radical cystectomy.The pathological stage of 79.7%patients focused on Ta and T1.2.The score of life of quality and the current status of life of quality of patients with different demographic variables and clinical featuresThe total score of quality of life after operation in bladder cancer patients was 74.70 ± 11.51,with the score of specific modules better than common modules.In the four fields of the common modules,the scores from low to high were:physical function,psychological function,social function,and common symptoms and side effects.The scores of specific module from low to high were:sexual function problems,psychological function problems,urinary tract symptoms,urinary bag problems,and specific somatic symptoms.The patients with different occupation,educational level and family economic status had different postoperative quality of life,the scores of life quality of worker and farmer were significantly lower than them of cadres,teachers,individuals and other occupational patients.The patients with undergraduate and above education showed better quality of life than other academic patients.The patients with better family economic status had higher quality of life.3.The score of hope level of patientsThe total score of hope level after operation in patients with bladder cancer was 37.81±3.32,which was at a high level.In the three dimensions of the hope level,taking positive action had the highest score,with positive for the reality and the future lowest.4.Correlation analysis of quality of life and hope levelPearson correlation analysis showed that the total score of quality of life was positively correlated with the total score of hope level(r=0.190,P<0.05)and its dimensions(except for positive for the reality and the future);taking positive action was positively correlated with the total score of quality of life(r = 0.240,P<0.05),and the physical function(r = 0.210,P<0.05),psychological function(r = 0.340,P<0.05)and social function(r = 0.244,P<0.05).Keeping intimate relationship with others was positively correlated with the total score of quality of life(r=0.184,P<0.05)and the psychological function(r=0.221,P<0.05)and social function(r=0.202,P<0.05)involved in common modules.Positive for the reality and the future had no correlation with the quality of life and its dimensions.5.Multivariate regression analysis of quality of lifeFirstly,taking the total score of quality of life acted as dependent variable,general demographic data and clinical features of the indicators,such as sex,age,education,family economic status,tumor staging,surgical methods and other indicators as independent variables,multiple linear regression analysis showed that the family economic status was the impact factor of the patient’s quality of life.Secondly,taking the quality of life acted as dependent variable,the total score of hope level and its dimensions as independent variables,analyze the influencing factors of the quality of life.The results showed that the family economic situation,the total score of hope level,keeping positive for the reality and the future were factors that influence the quality of life in patients with bladder cancer independently.Two regression equations revealed that the better the family economy,the higher the total score of life.Meanwhile,patients with higher score of hope level and more positive attitude of the reality and the future had the higher score of the quality of life.Conclusions1.The overall quality of life of inpatients with bladder cancer was better,which was above the middle level.2.The patients with different occupation,educational level and family economic status had different postoperative quality of life,the quality of life of worker and farmer was lower than that of the cadres,teachers,individuals and other occupational patients.The patients with undergraduate and above education had better quality of life than other academic patients.The patients with better family economic status had higher quality of life.3.The overall hope level of hospitalized bladder cancer patients was at a high level.4.There was a positive correlation between the quality of life and hope level in patients with bladder cancer.The higher the hope level came with the higher the quality of life.5.Family economic status,hope level and positive attitude towards reality and the future were significant influencing factors of the quality of life.Therefore,we suggest that more attention should be paid to these factors in the clinical care work,and take positive measures to improve the quality of life of patients. |
