ObjectiveThis study aimed to evaluate the quality of life of patients with colorectal cancer, hope levels and medical coping style, to analysis the relevance so as to improve the quality of life of patients with colorectal cancer, to help the nurses assist patients with colorectal cancer better ,to promote their physical and mental development, and to provide the theoretical basis of clinical nursing intervention.MethodsPatients with colorectal cancer in Henan Tumor Hospital are the target from September 2009 to January 2010 . The related questionnaire is divided into four parts: The first part is general information questionnaire ,which is to collect patients' demographic data and clinical and pathological data; the second part is the measurement form of patients' living standard, with the adoption of quality of life questionnaire-core 30,QLQ-C30 designed by the European Cancer Organization for Research and Treatment (EORTC) ; the third part is the measurement form of their hope, with the adoption the Herth (Herth Hope Index, HHI) Hope Scale translated by Zhao Haiping; the fourth part adopts MCMQ (Medical Coping Modes Questionnaire) by Feifel H to measure patients' .150 questionnaires were handed out and taken back with the effective rate of 100%. All the data were processed with SPSS13.0 software. The statistical methods includes: descriptive statistics, ANOVA, t-test, Pearson correlation, multiple linear regression analysis.Results1. Patient quality of life of patients with colorectal cancerTheir overall quality of life got 50.3±20.8 points (out of 100 points), belonging to the middle and lower level. In the functional areas, the cognitive function tops with 70.0±26.8 points, followed by the emotional function and physical function with 68.3±20.6 points and 67.3±24.2 points, whereas the social function scores the lowest with 58.4±31.1 points. In the field of symptoms ,fatigue tops with 46.3±25.2 points. In the individual entries, economic difficulties got 55.1±33.6 points. 2. The level of hope of hospitalized patients with colorectal cancerThe overall hope level got 38.09±3.83 points, varying from 28 points to 48 points. The low level of hope got 0%, the medium hope 26 cases, 17.3%, and the higher hope level 124 cases, 82.7%, which suggests a higher overall hope.3. Coping modes of hospitalized patients with colorectal cancer150 cases of colorectal cancer patients use a variety of coping modes ,with "to face" the main coping style (58%), "to avoid" (31%) and "to yield" (11%).4. The effect of different individual characteristics on patients' quality of lifeANOVA analysis shows that quality of life of patients are affected to some degree by level of education, family income, the means of paying medical expenses, the stage of treatment, clinical tumor stage, metastasis and stoma .5. The correlation between patients' quality of life and their hope levelRelated analysis shows that the patients' overall level of hope and its three dimensions are positively correlated with the overall quality of life(r=0.179,0.201,0.196,0.178, P<0.05); patients' positive attitudes toward reality and future attitude are positively correlated with five functions of quality of life, including character function, cognitive function, and social function (r=0.193,0.191,0.208, P<0.05); patients' attitude to positive action is positively correlated with cognitive function and social function(r=0.203,0.266, P<0.05); patients' attitude to maintaining close relations with other people is positively correlated with role function, emotional function andsocial function (r=0.164,0.204,0.166, P<0.05) .6. The relevance between quality of life of patients and the medical coping modesThe related analysis shows that "to face" approach is positively correlated with their overall quality of life (r=0.216, P<0.05), and the same does for role function and social function(r = 0.199, 0.212, P<0.05)."To avoid" approach is positively correlated with emotional function in the functional areas (r=0.200, P<0.05)."To yield" approach is negatively correlated with their overall quality of life (r=-0.302, P<0.05), with the five functional parameters in the functional areas (P<0.05), with fatigue, nausea, vomiting and pain in symptom factors (r=-0.289, -0.198, -0.326, P<0.05), and with insomnia, diarrhea in individual entries (r=-0.166, -0.200, P<0.05) .7.Multiple linear regression analysis of factors affecting patients' quality of lifeMultiple linear regression analysis shows the result: 150 patients hospitalized colorectal cancer patients' overall quality of life and clinical pathologic stage and "yield" coping styles, and negatively correlated positively correlated overall hope level.Conclusions1. The overall quality of life of the150 cases of patients hospitalized with colorectal cancer is in the middle and lower level. ANOVA analysis showed that: These factors are general factors affecting the patients on their quality of life, such as the level of education, family income, medical expenses, the way of payment for treatment, the stage of treatment, clinical tumor stage, metastasis.2. The 150 cases of patients hospitalized with colorectal cancer make a high level of hope level, and the living quality of the patients is related to the hope level3. The 150 cases of patients hospitalized with colorectal cancer adopt a variety of way to cope, and the way of facing is adopting more. the living quality of the patients affect the way of facing in the positive way.4. Multiple linear regression analysis shows the result: 150 patients hospitalized colorectal cancer patients' overall quality of life and clinical pathologic stage and "yield" coping styles, and negatively correlated positively correlated overall hope level.
