Objective:The purpose of this study mainly according to the results of the clinical treatment and effect of the treatment of lower limb long bone fracture(femoral shaft and tibial shaft Fracture),by collecting a large amount of patients on clinical application of actual Bridge combined fixation system(BCFS),with large sample research of evidence-based medicine to evaluate its safety and effectiveness more scientifically in the clinical application of this system?Finally,we could provide more reliable internal fixation treatment for fracture patients through analyzing dates and improving technology constantly.Method:This study reviewed 89 cases of lower limbs long bone fracture in trauma tic orthopedic trauma ward of shandong provincial QianFo Shan hospital from May 2014 to December 2016,there were 59 cases met our requirement after screening,among them,there were 43 male cases,16 female cases;36 cases of femoral fracture,29 cases of tibial fracture,3 cases of femoral with tibial fractures,2 cases of bilateral femoral fractures,1 case of bilateral tibial fractures.Their biggest age was 79 years old,the smallest was 8 years old,the average age was 37 years old;The Causes of fractures respectively were high falling injury(7 cases),car accident injury(39 cases),crash injury(3 cases),falling injury(10 cases);According to AO long tube fracture classification,the type A(simple),16 cases,type B(wedge type)29 cases,C(complexity)20 cases;all of them have completed the operation and been followed up for a average period of 12 months(the minimum was 6 months,the longest was 18 months).They accepted surgery treatment in 2 hours to 14 days after injury,the average time was 3 days.All cases had no large bone defection and nerve vascular injury,in addition to the fracture they had no obvious other serious diseases.According to different modus operandi,We set bridge combined internal fixation as the treatment group,and general locking compression plate(LDCP)as normal control group.On the basis of preoperative assessment?clinical diagnosis and iconography to determine the operation time.Internal fixation were BCFS and DCP in the operation.We carefully protected the soft tissue and vascular nerves during surgery and noted operation time?amount of bleeding?soft tissue injury.To evaluate two kinds of fracture fixation,we analysis and compare two groups in age,sex,fracture type,causes,injury parts and operation duration,intraoperative blood loss,postoperative recovery time,postoperative complications,so as to better guide us to choose propel treatment measures in such fractures in future clinical work.Statistical MethodsAll the datas were showed by x±s and analyzed by SPSS19.0 statistical software.Comparison between two groups is analyzed by t-test and signed-rank test.All the analysis is with P<0.05 as statistical significance.ResultsThere was no obvious difference on the sex?age?fracture type?causes,injury parts in two groups.compared with the control group,amount of bleeding of treatment group decreased.compared with the control group,postoperative recovery time of treatment group decreased.compared with the control group,operation duration and postoperative complications of treatment group decreased.According to the Johner-Wruh evaluation standard,the excellent and good rate of the treatment group is higher than the control group.According to the analysis of postoperative fracture standard,fracture treatment group showed excellent and good results,and higher than the control group.ConclusionsCompared with ordinary steel,bridge combined internal fixation system is the more effective treatment for lower limb fracture internal fixation,it characterized by internal fixation with no direct compression on fracture part,reduce the periosteum stripping,protect the blood supply of fracture end,and is beneficial to fracture healing,reduce the postoperative complications.BCFS for elastic fixation,convenient operation,short operation time,small trauma,conforms to the modern view of minimally invasive surgery.Connecting rod and the connection piece shape prefabricated flexible and mobile,especially suitable for large bone crushing or defect of fixed anchor end bridge is put up and down.Bridge combined with internal fixation system individualized treatment method is in line with the latest concept,apply to the clinical,and worthy of further research.
