Objective:With the improvement of modern living standard, the transportationand the rapid development of real estate construction, leading toloss of high-energy sex more and more, because of the lower limbslong tubular bone anatomy and its special physiological function,long tubular bone fracture of lower limb is more and more getattention.The current medical level continuously improve,treatments tend to be diversified, but we failed conservativetreatment of long tubular bone fracture of lower limb in thesurgical treatment is given priority to, the choice of surgicalapproach to the traditional classic intramedullary fixed platefixed and give priority to.This article through to the long tubularbone fracture of lower limb, especially the upper section offracture in cases of intramedullary needle fixed data wereretrospectively analyzed, by comparing the closed reduction andlimited open reduction and intramedullary fixation for thetreatment of long tubular bone fracture of lower limb surgery timeand fracture healing time, to explore the diagnosis andtreatment/interlocking intramedullary pin the clinical effect ofthe treatment of long bone fracture of lower limbs.In order to themore common clinical fractures of the choice of operation method to make certain theoretical guidance for the first time, reduceintraoperative long operation time and postoperative patientswith prolonged bed of pain.Method:In this paper, through a retrospective study during April2012toAugust2014, the sino-japanese fellowship at jilin universityhospital orthopedic trauma group make a diagnosis and givetreatment of trauma to the lower limb,81cases of long tubularbone fracture patients, after screening the get53cases,including31cases of male, female21cases, including double lowersection of femur fractures in1case;Aged23-77, an average of47.5years.Selection criteria:53cases, because of the type Afractures with intraoperative closure failure, choose limitedopen reduction and interlocking intramedullary pin fixation in4cases, yu are B and C type fractures and shifting bone block in27patients with obvious.Causes traffic accident injury in31cases, falling injury11cases, crush injury3cases,8casesfall;Upper middle period of femur fractures32cases, uppersegment tibia fractures21cases,24patients with multipletrauma.According to the AO/OTA classification32type a in5cases,32b15cases,32c type6cases,42type a7cases,42b in11cases,9cases of42c type.To pay the interlocking intramedullary pin limited open reduction and internal fixation for treatmentgroup, closed repairs to the control group, the average follow-upof18months (minimum6months, the longest30months).All thefractures are fresh fractures, multiple trauma patients with localsoft tissue contusion is heavier, the vast majority of patientswith no obvious bone defect.Operating time for5hours to threeweeks after the injury, intraoperative to enlarge pulp, staticfixed[3].Statistical methods:Application of SPSS19.0statistical software analysis,measurement data with mean add and subtract standard (plus or minusS) said, using t test, counting data with χ squared, with P <0.05,says the difference had statistical significance.Results:Group B than in group A shorter operation time, intraoperativeblood loss less.Group B than in group A short fracture healing time, postoperativepatients with shorter time in bed.Group A and group B in fracture bone nonunion degree has nostatistical significance.Conclusion:Limited open reduction in lower limbs long bone fractures treated by interlocking intramedullary pin fixation with AO/OTAclassification is widespread, and intraoperative intramedullarypin can accurate placement and can accurately judge the fracture,significantly shorten the operation time and reduce the damagecaused by intraoperative C arm fluoroscopy, for patients withsevere bone defect need bone graft can issue for bone graft,reduction caused by second phase graft surgery of patients withtraumatic injury and economic burden of negative consequences, andto reduce postoperative infection caused by long exposure time ofincision, and reduce the incidence of bone fascia room syndromeoccurred, shorten the postoperative recovery time, is a safe andfeasible method.
