Objective: Pilon fracture of tibial is not so common in orthopaedics clinic, which is often occurred to adolescents and adults. It involve a significant portion of the weight bearing articular surface and overlying metaphysis are notoriously difficult to treat. They occur when the talus is driven up into the tibial articular surface. All of these give patients tremendous pressure on life and economics. This text discussed and studied its choice of fixation means and prevention and cure of complications. This research reviewed and studied Chinese and Western medical literature about Pilon fracture presented the treatment that used Ortherfix monolateral external fixator combined with limited internal fixation in conjunction with jieguyihaofang. Through the research, we found that Ortherfix Bonolateral external fixator combined with limited internal fixation was a better fixation means in a good many fixation means about this kind of fracture. So we choosed Ortherfix monolateral external f ixator combined with limited internal fixation to cure the fracture. Chinese medicine curing the bone fracture have got the long history, and at decreasing swelling to relieve pain, romotirig the bone fracture' s concrescence have already obtained the good curative effect. Jieguyihaofang can decrease swelling to relieve pain in the bone fracture earlier period, prevent and cure compartment syndrome, in the medium-term and terminal can promote the bone fracture concrescence.Method: This text established clinic observational tables for 20 cases of open tibial and fibular fracture to study and research. All cases were divided into cure group and control group(each group had 10 cases) at random. The cure group used Ortherfix monolateral external fixator combined with limited internal fixation in conjunction with jieguyihaofang and control group only used Ortherfix Bonolateral external f ixator combined with limited internal fixation. We researched and recorded the data of pain , tumidness and bone fracture concrescence.Result: The total efficiency rates of two groups both are 100%;the cure rate of cure group is 80%, which of control group is 70%. Through statistic disposal , the total efficiency rate' s comparison of two groups is P>0.05, and no significantdifference. The cure rate' s comparison of two groups is X2=4.389, P=0.036<0.05, so which expresses that the curative effect of cure group is better than that of control group after 3 months. The points' s comparision of pain, tumidness, callus growth before and after the cure has significant difference, which expresses that the curative effect of cure group is better than that of control group.Conclusion: Ortherfix nonolateral external fixator combined with United internal fixation is a effective fixation means of curing pilon fracture, jieguyihaofang has a good curative effect at decreasing swellinging to relieving pain, promoting the bone fracture' s concrescence.
