Objectives: To observe the effects of medical absorbable hemostatic gauze on visceral hemorrhage and tissue absorption in minipigs,and skin wound healing in rats.Methods : 21 cinnamon minpigs radomly divided into 3 groups,respectively medical degreasing cotton gauzes(normal gauze,NG)of 6 pigs,surgicels of 6 pigs and medical absorbable hemostatic gauzes(MAHG)of 9 pigs.Each animal was anesthetized with ketamine.The liver was exposed and made two wound surfaces,including the big one was 2.0cm×2.0cm,and the small one was 1cm×1cm,in the right lobe margin.The final autopsy made a 1cm longth incision in the spleen and mesenteric blood vessel,and then dealed with hemostatic material,for evaluting hemostasis time and the amount of bleeding.Jugular blood from each animal was collected at the time of pre-operation(0 day)and post-operation 1week,2week,4week,to measure hematology indices,biochemical indices and blood clotting indices.In addition,the study was evaluated the absorbing state of hemostatic material and general toxicity by morphological changes,through dissection 1/3 number animals in each group at the time of post-operation 1week,2week,4week..30 SD rats weighting 180-220 g,after unhaired the back on both sides of the spine,were made three circle incisions(diameter 1.5cm),and the upper left side as control studies,upper right side dealed with MAHG,low right side using surgicel,included two layer gauzes.Continuously observe the wound healing status for 21 days,and at the time of pre-operation(as 0 day)and post-operation 4,7,10,14,21 days take pictures,measured the wound area and healing rate,and at the above observation time point,draw the wound region material to make a HE stain for evaluation of the quality of the wound healing situation.Results :Minipigs have no abnormality circumstance during the test,including food intake,drinking,body weight,and none of wound infection fester.The other indexes: hemostasis,absorbtion,hematology and biochemical index,state as follow.1 Hemostasis time and amount of bleeding of minipigs: MAHG and surgicel caused a significant decrease in hemostasis time of liver,spleen,mesentery blood vessle,compared with the normal cotton gauze(P < 0.05).Meanwhile,MAHG and surgicel treatment markedly decreased the amount of bleeding,compared with the normal cotton gauze group(P < 0.05).There are no significance with MAHG and surgicel.2 The liver absorbtion of gauze: There are obviously vestigital in NG group during 2 weeks,accompany with inflammatory response,then the gauze form a layer of diolame at the fouth week.By contrast,the MAHG and surgicel appeared progressive absorbtion following with the time expansion compared with NG material,and the liver completely absorbed the material.There was no obviously difference with MAHG and surgicel groups,on inflammatory response and healling situation.3 Hematology,biochemical and blood clotting indexs: There were increase MO%(the 2nd week),ALT(the 4th week)after operation in NG group.Meanwhile,surgel group decreased HCT(the 1st week),HGB(the 1st week),and obviously increased in TP(the 4th week),T-CHO(the 4th week)at post-operation,the changed of these indexs may have relation to the operation hemorrhage or the irritation of liver.MAHG group decreased in PLT(the 2nd week),GLU(the 2nd week)and BUN(the 4th week),other indexs were normal,the index of lower BUN had no biology meaning,the lower GLU may have relation to the less diet.4 The liver histopathological examination: NG group had a serious granuloma,vestigital,infammatory,adhesion and infection.This group had no benign change in granulation tissue with the time extension.The MAHG and surgicel groups have an obviously alleviation in histopathological change,there are no infection,although two groups have a gauze vestigital,there are no obvious granuloma formation.MAHG group have a more relieve in amount of residue,inflammatory cell infiltration,adhesion following with the longth of time.5 The skin wound healing status: There were no serious infection,fester,effusion in wound region area 1.0cm2 excepted a little errhysis after operation.The surface of full wound skin healed slowly at the 4 days,fastly during 4~10 days,and completely healed at the 21 th day.Comparing the MAHG and surgicel groups to sham operation group have no signifcant difference(P >0.05).6 The skin histopathological examination: The pathological charactristics of traumatic skin is to form granulation tiss and fibrillogenesis.Three groups have no difference in the healing time phase,there are a little of crusta during 4~7 days,less more and more crusta in 10 th day,and completely disappear in 21 st day,epidermis completely cover the wound region.There are local granulation tissue fromation in the 4th day and is obvious in 7~10 days,after that there are more and more collagenous fiber formation.Three wounded skin completely healed in the 21 st day.To compared with sham-operation group the MAHG and surgicel groups have no difference on delay the wound healing,also hits MAHG have no effect on the wound healing.Conclusion :MAHG have a function in preventing visceral hemorrhage,and can be absorbed by liver tissue progressively.Moreover,MAHG have less vestigital and foreign body reaction than surgicel.Furthermore,MAHG has no effect on wound healing in rat's skin injury.
