| PurposeThis study followed up by clinical observation of the patients,and compared with the positive control drug Weifuchun randomly.Based on the trial and matched group in scores and improvements of clinical symptoms and the main symptoms,improvement of individual major symptoms,endoscopic gastric mucosal appearance' s conversion,the pathological changes and the rate of disappearance of three different parts of the stomach antrum,stomach angle and stomach body as well as the Chinese version of SF-36 scale eight dimensions of the points to a more comprehensive evaluation and analysis of addicted to the stomach side of the Clinical efficacy and pathological improvement of atrophic gastritis with precancerous lesions.So as to evaluate the clinical efficacy of the hospital preparation-modified preparation of the decoction in a more objective and comprehensive way.MethodA total of 80 cases patients which meet the inclusion criteria suffer from CAG with precancerous lesions(For HP infection to quadruple therapy,anti-HP therapy treatment)in Guangdong Second Hospital of traditional Chinese Medicine from September 2014 to September 2016,and it was taken by a randomized controlled study.The test team of 40 cases treated with modified unripe stomach decoction,ldose/day,after morning and evening meals;the matched group for 40 cases which take Weifuchun,4 tablets/time,3 times/day,also after meals.After 6 months of treatment,the patients return to our hospital for further consultation to reexamine gastroscopy,pathology,etc,for the purpose of analyzing the clinical symptoms,the gastric mucosa under endoscopy,pathological changes of gastric antrum,gastric angle and gastric body,and the quality of health scale after treatment.So as to make a comprehensive and analysis of its clinical efficacy objectively.ResultsClinical observation results:1?symptomatic efficacy:The total effective rate of the trail group and the matched group were 95.00%and70.00%respectively,and the effective rates were 85.00%and 37.50%respectively.There were big differences between the two groups(P=0.010,0.010<0.05).And the total effective rate and the apparent effective rate(97.50%,95.00%)of the experimental group were significantly higher than those of the control group(70%,47.50%),and the difference was significant(P=0.000,0.000<0.01).Two groups of patients with total symptom scores and the main symptom scores were lower than pretherapy,and the test term was significantly lower than the matched group(P=0.000,0.000<0.01).There were differences between the two groups' efficiency of individual symptoms in major symptoms:in the stomach pain,bloating,belching and anorexia,the cure rate of test term was 88.57%,93.94%,92.00%and 77.78%respectively higher than that of the matched group namely 65.63%,70.97%,65.38%and 38.89%,and the effect of the test team was better than that of the group(P=0.039?0.021?0.038?0.041<0.05).2?Improvement of gastric mucosa under gastroscope(The main observations of it are the thickness of gastric mucosa,mucosal color,vessel can be seen or not,mucosal roughness,etc.).The scores of two groups were lower than pretherapy(P=0.000?0.000<0.01).In addition,the effect of gastroscope in the treatment group was significantly lower than that in the matched group(P=0.000<0.01).3?Pathological improvement:?Six months later,the two groups' pathological effects in the gastric antrum,gastric angle,gastric body of the three parts of the atrophy?intestinal metaplasia and dysplasia were improved compared with pretherapy.The test team was significantly better than the matched group(P<0.05).Compared with the control group,the improvement of chronic inflammation is better in gastric antrum and gastric angle(P<0.05),but there was no significant difference in the therapeutic effect of the body parts(P>0.05).The two groups can improve activity,and there was no significant difference between them in the therapeutic effect(P>0.05).?In the atrophy of gastric antrum,gastric angle and gastric body,the cure rate and apparent efficiency of the treatment group is better than the control group(P=0.005?0.005,0.003?0.005,0.001?0.003<0.05).In the total efficiency of gastric body,the treatment group is better than the control group(P=0.014<0.05),and in the gastric antrum and gastric angle,there is no significant difference between the two groups(P=0.066,0.115>0.05).In intestinal metaplasia,dysplasia and chronic inflammation aspects,the experimental group was better than the control group in the cure rate,apparent efficiency and total efficiency.(P=0.044?0.000?0.000,0.006.0.006?0.019,0.005?0.005?0.005;0.044?0.000?0.000,0.000?0.000?0.000,0.042?0.042?0.042;0.021?0.000?0.000,0.001?0.001?0.000,0.035?0.035?0.035<0.05).The cure rate,apparent efficiency and total efficiency of the activity are no significant difference between the two groups(P=1.000,1.000,0.448;0.073,0.073,0.066;0.139,0.139,0.128>0.05).?In the test team,the cure rate of atrophy,intestinal metaplasia,dysplasia,chronic inflammation of pathological aspects is better than the other group(P<0.05).However,in the activity,there were no difference between the two groups(P>0.05).4?Two groups of patients in the SF-36 quality of life score:Two groups of SF-36 scale in 8 dimensions is better than pretherapy(P=0.004?0.000?0.000?0.000?0.000?0.000?0.000?0.000<0.01).The therapeutic effect of BP and SF in the test team was better than that in the matched group(P=0.000,0.040<0.05).There was no big difference in quality of other six dimensions between the two groups(P>0.05).ConclusionThe results of the treatment of chronic atrophic gastritis with precancerous lesions in symptoms,endoscopic mucosal tissue,the shrink,intestinal metaplasia,and dysplasia of gastro in different parts of atrophy are effective and significant.What is more,it can improve the life quality of patients in different degrees.Thus it is worthy of further study and clinical application. |
