| Objectives Compare the therapeutic effect of different methods(fire needle combined with Electroacupuncture(Electroacupun-cture,EA),EA,medicine)treating herpes zoster,and determine the effect of them on serum(IL-4,TNF-a)content changes before and after treatment,using randomized controlled observation.We aim to explore the possible mechanism between the changes of cytokines and acupuncture analgesia and provide clinical evidence and a more favorable treatment plan for clinical acupuncture and moxibustion treatment of HZ.Methods 1.From January 2014 to December 2016,40 cases consistent with the diagnostic criteria,inclusion criteria were recruited as the research object from inpatients of Wuhan Central Hospital affiliated to Huazhong University of Science and Technology and outpatients and inpatients Combined Traditional Chinese and Western Medicine Hospital affiliated to Huazhong University of Science and Technology.2.By random,the subjects were divided into fire needle plus EA group(12 cases,treated with fire needle combined with EA therapy)and EA(12 cases,treated with EA)and Western medicine group(a total of 16 cases,treated with Western Medicine).Three groups of patients were treated once daily,and 10 consecutive times were 1 course of treatment.They were treated for 1 course,and the curative effect was evaluated after the treatment;3.Evaluate pain index.The time when pain began to ease time and the duration of pain relief;4.Evaluate symptom and sign quantization score.Score the symptom and sign quantization score at the first time before treatment,and after the tenth treatment;5.The serum samples were collected and analyzed.The levels IL-4 and TNF-a of serum from patients were measured before the first treatment and after the tenth treatment respectively;The above observation data were analyzed by using SPSS 19.0 statistical software.Results 1.General information.We compare the gender,age,body temperature,duration,the time when the pain appear before treatment and the time when blisters appear of the 40 patients by statistical analyses.We found that the difference between them has no statistically significance(P >0.05);2.The curative effect comparison.According to clinical criteria,the effects of three methods on patients before and after treatment were calculated and analyzed.In EA group,1 case was cured,3 cases were conspicuously effective,6 cases were relatively effective,no ineffective case.The total efficiency was 83.33%.In fire needle plus EA group,3 cases were cured,6 cases were markedly effective,3 cases improved,0 case was ineffective,the total efficiency of 100%.In the western medicine group 0 case was cured,4 cases were conspicuously effective,7 cases were improved,5 cases were ineffective,the total efficiency of 68.75%.By the analysis of variance and Ridit analysis,we compared the comprehensive curative effect of the three kinds of methods.We found that the effect of the fire needle and EA group was better than the EA group and Western medicine group(P <0.05).There is no significant difference on the curative effect between the EA group and the Western medicine group(P >0.05).But in the total efficiency,the EA group was better than the western medicine.3.The pain index comparison.Three groups of patients had significant difference in the time when pain began to ease and duration of pain relief between groups(P <0.05).Compared with the western medicine group,the time when pain began to ease and duration of pain relief was shorter in the EA groups and fire needle plus EA group(P <0.05).Compared with the EA group,the time when pain began to ease and duration of pain relief were shorter in the fire needle plus EA group(P <0.05).4.Quantification of symptoms and signs score scale.There is no significant differences among the three groups before treatment(P >0.05).Comparison of the three groups before and after treatment,the differences were statistically significant(P <0.01).It indicated that all the three methods could significantly improve the symptoms and signs.After the comparison between groups after treatment,the effect of the fire needle plus EA group on improving the symptoms and signs is better than the western medicine group and the EA group(P <0.05).There was no significant difference between the western medicine group and the EA group(P >0.05).5.Determine the level IL-4 and TNF-a of serum.There was no significant difference before and after treatment of three groups,and also between the groups in the level of serum IL-4 in comparison(P >0.05).In the serum level of TNF-a,there were significant differences before and after treatment in fire the needle plus EA group and the western medicine group respectively(P <0.05).There was no difference before and after treatment in the EA group(P >0.05).Comparison of data between groups,there was no difference among the three groups before and after treatment in the level of TNF-a(P >0.05).Conclusions 1.Compared with the comprehensive curative effect and improvement of the signs and symptoms of HZ patients among three kinds of treatment,the fire needle plus EA group is better than in the EA group and the Western medicine group;no difference between the EA group and the Western medicine group on curative effect.But in the total efficiency,the EA group was better than the western medicine.2.The fire needle plus EA group was the shortest at the pain began to ease pain time and the shortest duration of pain relief.The EA group was at the middle one.The western medicine group was the longest one.3.The level of the TNF-a between the fire needle plus EA group and the western medicine group after treatment is lower than before treatment.The EA group had no difference in change before and after treatment.The level of TNF-a had no change in meaning after treatment among the three groups.The level of IL-4 had no significance before and after the treatment in groups and between groups. |
