Objectives: By comparing with small needle knife with sodium hyaluronate and arthroscopic treatment the curative effect of early metaphase knee osteoarthritis(KOA)and cost studies,try to explore a kind of to the lowest costs to achieve the optimal treatment of early metaphase KOA diagnosis and treatment plan,guidance and reference for clinicians and patients with diagnosis and treatment,so as to better implement the modern medical mode,saves medical cost,harmonious doctor-patient relationship.Methods: Will be December 2013-December 2015 in a 3 a certain hospital treatment during the period of middle 120 cases of KOA patients were randomly divided into 2groups,treatment group 60 cases of patients with small needle knife sodium hyaluronic acid treatment,the control group 60 cases of patients with arthroscopic treatment;Were observed to compare two groups before and after treatment in patients with 5 weeks VAS pain score,2 groups of patients 12 weeks after treatment the clinical curative effect,the knee joint symptoms Lequesne index score before and after treatment in patients with 12 weeks,the changes of two groups patients hospitalized days,hospitalized period cost,recovery cost,total cost,analysis of the cost of hospitalization period,recovery cost,total cost,and the correlation of knee joint function score difference between before and after treatment;Compare two groups patients with health care and the health care cost of hospitalization period,the number of days in hospital.Results: 2 groups of patients before treatment VAS pain score,the knee joint symptoms Lequesne score comparison differences had no statistical significance(P > 0.05);Two groups after treatment in patients with 5 weeks VAS pain score were significantly lower compared with before treatment,respectively,the difference was statistically significant(P < 0.05),5 weeks treatment group after treatment VAS pain score was significantly lower than control group(P < 0.05);Treatment group total effective rate was 93.3%,control group total effective rate was 86.7%,the efficient there was statistically significant difference comparing the two groups(P<0.05);2 set of 12 weeks after treatment in patients with knee joint symptoms Lequesne score were significantly lower compared with before treatment,respectively,the difference was statistically significant(P<0.05),treatment group 12 weeks after treatment knee joint symptoms Lequesne score was significantly lower than control group(P<0.05);Among them 12 weeks after treatment the treatment group compared with control group,the tenderness,swelling,Lequesne score differences no statistical significance(P>0.05),and in the rest pain,pain,morning stiffness,ability to walk on Lequesne score difference was statistically significant(P < 0.05);The treatment group patients average hospitalization days was 13.5+/-1.96 days;The control group patients average hospitalization days was 9.08+/-2.37 days;2 groups of patients hospitalized days comparative difference was statistically significant(P<0.05);2 set of hospitalization cost comparison difference was statistically significant(P<0.05),was in the hospital for the treatment group than controls;Two groups patients convalescence cost comparison differences had no statistical significance(P>0.05);Two groups total cost comparison difference was statistically significant(P<0.05),the overall cost of the treatment group is lower than the control group;Treatment group hospitalization days and the period of hospitalization expenses in the 2 groups,rehabilitation period cost,total cost,before and after treatment to the knee joint function score difference of correlation analysis has significant correlation(r>0,P<0.05);The control group before and after hospitalization days and treatment of knee joint function score difference correlation analysis has no obvious correlation(r>0,P>0.05);2 groups patients hospitalized period expenses,hospitalization days were higher than patients without health insurance,compare the difference was statistically significant(P<0.05).Conclusion: Small needle knife with sodium hyaluronate and arthroscopy two middle treatment of KOA treatment has better clinical curative effect,small needle knife together with the sodium hyaluronate in middle ease of KOA patients rest pain,pain,morning stiffness,the ability to walk on symptom has better curative effect,and the more safe,economic andminimally invasive treatment,can effectively reduce the economic burden of patients and psychological burden.Of sodium hyaluronate and as a result,small needle knife for the treatment of early metaphase KOA has better curative effect,the cost is worth clinical application.
