| Knee Osteoarthritis(KOA) belongs to osteoarthritis.KOA is a kind of chronic progressing arthrosis disease,of which incidence is on a steady increasing trend along with longer life span and increasing number of old people.It is also called knee hyperplasia arthritis,retrogressive arthritis, hypertrophic arthritis,retrogressive osteoarthritis.Knee osteoarthritis is one of the most commonly encountered orthopedics diseases which is especially common to the olds.Its main clinical manifestation is progressing chronic Joint pain,rigidity,intumescences accompanied with movement restriction,which can lead to dysfunction of joint,even deformity in severe cases.It' s the main reason of elders' joint ache and disability.In 1995,a new definition was released in the international meeting for OA,that is,OA is the result of disequilibrium of decompose and anabolic metabolism among cartilage cell,ECM and cartilage sending down fishbone,caused by the integration of mechanical and biological factors.The IL-1,6,TNF-a,NO which found in the molecule biology area are the most important factors for the cartilage dandification.In the Biology mechanics fields,the increase of the bone' s inside pressure is thought as an important reason for arthrosis.The KOA is usually adscripted to arthromyodynia or rheumatism involving bone,usually identified as deficiency of the liver and kidney larding with kinds of the asthenia of evil,or deficiency of QI and blood larding with cold and phlegm.Treating it with taking Chinese medicine,washing,acupuncture,massage or complex treatment,can get curative effect.This research consist a summary of clinical literature and results obtained from clinical observation.In the document reviews,the modern medicine and traditional Chinese medicine comprehension to knee osteoarthritis is discussed. Modern research on knee osteoarthritis is summarized from etiology,pathology, pathogency,treatment.At the same time,actuality of Chinese medicine treatment to knee osteoarthritis is reviewed.From the fruits of recent years,the balance of joint stress isdiscussed deeply.Balance of joints tress is considered one of the most important factors in the pathogency.Clinical researchMian purpose:To study clinical therapeutic effect and safetyof acupotomy.Method:All patients were selected from those who came to visit traditional Chinese medicine department of Naval General Hospital of Beijing between 2008.3-2008.10.The total number was 60.They were classified into two groups at random.One was management group,30 patients were treated with acupotomy. Another was control group,30 patients were treated with Antra-articular injection of sodium hyaluronate 20mg/2ml.Patients of management group were received acupotomy treatment fortnightly interval.Continuous course treatment was five weeks.Patients from control group were received Antra-articular injection of sodium hyaluronate 20mg/2ml every week.All patients were evaluated after five weeks.Result:Analyzed the data with statistics,we found the effective rate of management group was 93.3%,but control group 73.3%.Distinct difference was between two groups(p＜0.05).The curative effect of management group was superior to that of control group.Analyzed soreness scores and symptom scores between circa treatment with statistics(P＜0.05),we also found management group was superior to that of control group.Comparing staging of pathogenetic condition advancement and hierarchical pathogenetic condition by rank sum test,the result is all the same,especially in the primary and intermediate hierarchy.Management group also shows higher improvement than the control group in ambulation-odynia, stand assistance,Through the research we consider the short-term effect of treating knee osteoarthritis with acupotomy is favorable.And the therapeutics is worthy to be extended. |
