Objective:Hydrate of chlorine aldehyde is a common sedative for various auxiliary examinations in infants and young children. In clinical application, many factors can affect the hydration of chlorine aldehyde on infants and young children to achieve the desired effect of retention enema. This study was based on the clinical observation both of traditional methods and improved methods for the retention enema of hydrate in a hospital of Changchun City, then compared sedative and hypnotic effect before and after the improvement of retention enema of infants. The results of this study would provide theoretical basis for the application of the hydration of chlorine aldehyde in pediatric clinical work, and to provide theoretical basis for improving the quality of pediatric nursing.Method:300 subjects in the emergency department of a hospital of Changchun City were included for clinical observation object, who all meet the inclusion criteria that the age of these children in infant 0~3 years old from October 2014 to September 2015.The relevant information was obtained by questionnaire, and the clinical observation was divided into control group and improved group according to the principle of informed consent and random sampling. 150 cases in control group which adopted the traditional retention enema method: traditional catheters, 18 ℃ of enema, free body position. 150 cases in improved group which adopted different types of catheters, 25℃ enema temperature, low hip position lying on the left side and deepen the depth of insertion.By comparing with the traditional group and the improved group on catheter insertion depth, the change of the position before and after the enema, and the temperature of enema liquid, analyze the effect of retention time of retention enema, as well as the effect of retention time of hydrate on the onset and sleep time of infants.The data were collected by EXCEL 2007 and were analyzed by SPSS 20.0 software. P <0.05 was employed by statistical significance.Result:1. The comparison results of the basic situation of the traditional and the improved group are showed: Before and after the improvement, there were no significant differences in male and female composition, age and weight difference between the two groups(150 cases).2. The comparison results of the basic situation of the traditional and the improved group are showed: before and after the improvement of the two groups in the first enema, whether no history of drug allergy, defecation, whether using Suppositories Glycerol and intestinal abnormal differences were no statistically significant(P>0.05).The use of the catheter model in traditional group are 12, the improved group use the NO. 6, 8, 10 and 12 catheter model, the ratio was 46%, 18%, 19.3% and 16.7%, respectively. The temperature of the liquid medicine in traditional group is 18℃, and in the improved group was 25 ℃. The results showed the retention time and the onset time of sedative and hypnotic effects were lower than those of the modified before and after the administration(P<0.001). The improvement of the sleep time was lower than that of the modified anterior(P<0.001).3. The insertion depth of the time effect of retention enema after drug analysis of the insertion depth and the perfusion of retention time after analysis shows that: there is a positive correlation between the two factors(r=0.629, P<0.001), which means that increases with the depth of cannula, perfusion after retention time increased. Additionally, before the improvement, the position of the subjects was taken as free position.4. From the results, we knew that the position of the enema in traditional group was taken as free position, the improved enema position for the first sit after the left side(55.3%) and the left lateral position(44.7%).5.Effect of temperature on retention time of enema:We used different temperature of the liquid medicine, 25 ℃ Celsius after the filling time of the retention time is higher than 10 ℃. The difference is statistically significant(P<0.001).6. We used different methods to carry on the research object to carry on the sausage, and analyze the relationship between the duration of retention time and the onset time of sedation and hypnosis and the sleep time of the sedative and hypnotic agents, we found that there was a positive correlation between the retention time and the onset time of sedative hypnotic effect, and the difference was statistically significant(r=0.394, P<0.001).Meanwhile, there was also a positive correlation between the retention time and the sleep time after the administration of the drug, and the difference was statistically significant(r=0.637, P<0.001),which we considered that with the increase of retention time after irrigation, sedative hypnotic action time to speed up, and the sleep time decreased.Conclusion:The following conclusions were obtained by improving the catheter materials, the retention time, the insertion depth of the catheter, the change of position and the temperature of the enema fluid before and after the enema:1. Choosing the appropriate type of catheter can increase the retention time of the hydrate.2. Increasing the depth of catheter insertion can prolong the retention enema time of hydrate.3. Before and after the enema, the left position and the free body position were in favor of the retention time of the hydrate.4. The retention time of the hydration chlorine aldehyde temperature 25℃ is better than that of the 18 ℃.5. With retention time increased, the onset time and sleep time were increased.
