Clinical Observation Of Ulcerative Colitis Patients Treated By Retention-enema According To Syndrome Differentiation

BackgroundUlcerative colitis, which was named chronic nonspecific ulcerative colitis, was one type of chronic bowel diseases of unknown etio-logy. The pathological changes, which were mainly confined to mucous membrane and submucosa in the large intestine, usually involved rectum or distal colon, and they might extend to proximal or the entire colon. Major clinical manifestations included diarrhea, stool with grume and bloody purulent, stomachache and tenesmus. It had prolonged course, repeated attack, exacerbation by years and long treatment not to recover. Ulcerative colitis, which was difficult to cure and has high recurrent, had a certain canceration rate and was listed one of modern refractory dieeases by the World Health Organization.PurposeBased on the curative effect of retention-enema according to syndrome differentiation and rich clinical experience of pro. Chen in the treatment of ulcerative colitis, the study made recipe according to macro syndrome differentiation with microscopic appearance. Observed be-and-af treatment the changes of symptoms, that included stomachache, diarrhea, stool with grume and bloody purulent and intestinal mucosa damage. Provide the reference for clinical treatments.Patients and MethodsCollect 60 ulcerative colitis patients from coloproctological ward or outpatient department of Zhongshan Hospital affiliated to Guangzhou University of Chinese Medicine during October 2008 to February 2010, major violations of obstructing left-sided colonic lesions, aged 18-65 years old, and syndrome differentiated of the intense damp due to deficiency of spleen or the inside damp-heat type. Divided into two groups randomly,30 cases for treatment group and 30 cases for control group. The treatment group was given retention-enema treatment with chinese medcine according to syndrome differentiation, and the control group with Sulfasalazine 2g+Saline 200ml. Investigated each patient with a questionnaire and by video endoscopic examination before the treatment, and got a score. After one period of treatment, made the questionnaire and the examination again, got another score. Then analysis the data. All data were using the statistical software SPSS13.0, and difference analysis using t-test.Resultsdiarrhea, stool with grume and bloody purulent, stomachache and tenesmus.1, The curative effect of diarrhea was better in the treatment group than in the control group, and the change of their scores had extremely significant difference (P<0.01).2, The curative effect of stool with grume and bloody purulent was better in the treatment group than in the control group, and the change of their scores had extremely significant difference (P<0.01).3, The curative effect of stomachache was better in the control group than in the treatment group, and the change of their scores had extremely significant difference (P<0.01).4, The curative effect of tenesmus was better in the treatment group than in the control group, and the change of their scores had extremely significant difference (P<0.01).5, The treatment group had 30 patients,0 patient was healed,4 patients had good effect,25 patients had effect and only 1 patient had bad or no effect. So the overall efficiency was 96.67%. The contrl group had 30 patients,0 patient was healed,0 patient had good effect,23 patients had effect and only 7 patients had bad or no effect. So the overall efficiency was 76.67%.Conclusion1, Retention-enema with chinese recipe, which was according to macro syndrome differentiation with microscopic appearance, could greatly improve the signs and symptoms of ulcerative colitis of the intense damp due to deficiency of spleen or the inside damp-heat type. 2, Retention-enema with chinese recipe, which was according to macro syndrome differentiation with microscopic appearance, could greatly improve the intestinal mucosa damage of ulcerative colitis of the intense damp due to deficiency of spleen or the inside damp-heat type.
