Objective:Compare the curative effect between the gel of Chinese herb(GCH) and the tincture of Chinese herb(TCH) on soft tissue injury through infratest on rats and test in hospital, and observe the adverse effects on liver and kidney and and do some research about the mechanism at the same time. Seek for a more convenient form of Chinese herb to promote the development of Traditional Chinese Medicine. Decrease the pressure of medical care on patients and our country.Methods: For the infratest, totally 36 male SD rats were randomly divided into 4 groups:Normal group(Z), control group(C), tincture group(D), and gel group(N), each group has 9 rats. The rats in the group C, D and N were got injured on the right gastrocnemius by an iron ball. After 7 days of the injury, the rats in group D and N started treatment. For group D, we used the TCH wetting a piece of gauze about 2*2 cm2, then covered the wounded part with the gauze 1 hour a day. For group N, we covered the wounded part with a 2*2 cm2 gel paster of the GCH 1 hour a day. Before the treatment and after the 5th, 10 th treatment, blood and gastrocnemius samples were collected. The level of interleukin-1?(IL-1?) and prostaglandin E2(PGE2) in serum was evaluated by ELISA, the concentrations of alanine aminotransferase(ALT) and blood urea nitrogen(BUN) were determined by biochemical assay kits, the gastrocnemius samples were embedded in paraffin, cut and stained using the standard HE method, then do the histological examination.For the test in hospital, patients of neck, waist and knee were randomly divided into 2 groups: Tincture group(DJ) and gel group(NJ). Patients of DJ group were treated by covering the wounded part with the gauze wetted of the TCH, heating 30 minutes with infrared. Then do massage around the wounded part 10 minutes. Patients of NJ group were treated by infrared heating 30 minutes and 10 minutes massage, and then paste the GCH about 6-8 hours. Before the treatment and after 5 and 10 days of treatment, every patient would do a questionnaire to evaluate the level of diseases. The NDI score was for the patients of neck, JOA score was for the patients of waist, Lysholm score was for the patients of knees, and VAS score was for every patient.Results:1) The NDI and VAS score of group DJ and NJ were decreased after 5 and 10 days of treatment. The JOA and Lysholm score of group DJ and NJ were decreased after 5 and 10 days of treatment.2) After 5 days of treatment, the NDI score of group DJ was significantly decreased when compared with group NJ(p < 0.05), the JOA score and waist VAS score of group DJ was significantly increased when compared with group NJ(p < 0.05). As for other scores, there was no significantly between group DJ and NJ(p>0.05).3) The level of IL-1? and PGE2 from group D and N were decreased after 5 and 10 days of treatment, but this did not reach statistical significance by student's t test between group D and N(p>0.05).4) Compared with group C, the histological examination of gastrocnemius samples from group D and N were obviously improved after 5 and 10 days of treatment.5) The concentrations of ALT and BUN from all groups were at normal level, and the ALT from group N was significantly lower than that of group Z, C and D(p < 0.05).Conclusions: There is no obviously difference of curative effect between the GCH and the TCH on soft tissue injury, the mechanism is reducing the level of IL-1? and PGE2 and promoting the recovery of the injured tissue, but the GCH is worth to popularize because of its more convenient to use than traditional forms.
