| Objective:To observe the effect of treatment method of Wen yang fu zheng xiao ying fang to Autoimmune Thyroiditis in terms of clinical efficacy, related symptom improvement and inspection data analysis, provide the basis for the clinical application of the party, in order to provide new method and thinking for the treatment of the disease.Methods:1) The object of study:collect in March 2013 to June 2014 in Wuxi traditional Chinese medicine hospital outpatient surgical diagnosis and treatment of 45 cases of autoimmune thyroiditis (spleen kidney Yang deficiency type), through the principle of randomized, parallel-group,45 cases were randomly divided into Chinese and western medicine treatment group, Chinese medicine treatment group and western medicine treatment group,15 cases in each group.2) Treatment methods:traditional Chinese medicine treatment group:Wen yang fu zheng xiao ying fang 100 ml, twice a day. Chinese and western medicine treatment group:Wen yang fu zheng xiao ying fang 100 ml, twice a day;Levorotatory thyroxine pieces, 50?g, once a day. Western medicine treatment group:Levorotatory thyroxine pieces, 50?g, once a day. The treatment course are all for 3 months.3) Clinical outcome indicators and decision criteria:a clinical signs and symptoms: fatigue, aversion to cold, and cold limbs, waist and knee pain, not sweet, nocturia qing long valley, goiter, thyroid gland, thyroid nodules, texture and calculate the clinical symptom integral scale (refer to "Chinese medicine new medicine clinical research guiding principle");B (ICMA) by immune chemiluminescence method detection TT3, TT4, FT3, FT4, TSH, TGAb and TMAb. Each visit before treatment, fasting blood samples and record the test results;C reference Gao Xu wen standards.*:symptoms improved markedly, and thyroid nodules becomes soft small (>70% drop in clinical symptom score values), TG-Ab or TM-turn Ab or drop down> 50;Effective:symptoms improved clinical symptoms (total score value falls 30-70% or higher), TG-Ab or TM-Ab drops down from 20 to 50 or more; Invalid:did not meet standards effectively.4) Statistical methods:all the data after finishing use Excel to establish database, using SPSS 17.0 statistical software, statistical analysis, statistical description to the average± standard deviation, the median (interquartile spacing) or percentage, comparison between measurement data set using analysis of variance and rank and inspection, count data using analysis of variance, with a= 0.05 as the inspection level, P< 0.05 for the difference was statistically significant.Results:1) 45 patients with AT, all patients were followed up, no case stop studying. Three groups of patients with age, gender ratio and course of disease, showed no significant difference. (P>0.05);2) In general, Chinese and western medicine group and traditional Chinese medicine (TCM) clinical signs scores decline are more apparent, and the western medicine group decline more gentle; Chinese and western medicine group had significantly lower after treatment 2 w, Chinese traditional medicine group after treatment had significantly lower 8 w; Three groups in clinical symptom score comparison before and after the treatment are significant difference (p<0.05);Subgroup analysis, Chinese and western medicine and Chinese medicine group after treatment to cold, fatigue, and cold limbs valley not sweet symptoms were improved significantly.3) Thyroid hormone and Thyroid autoantibodies:Thyroid hormone:three groups of patients in TSH are improved to varying degrees after treatment (p< 0.05), one of them Chinese and western medicine group and western medicine group decreased obviously, Chinese traditional medicine group no significant decline; Thyroid autoantibodies: Chinese and western medicine and Chinese medicine group after treatment in patients with TGAb, TMAb value are reduced, and the Chinese and western medicine group, the difference was statistically significant before and after the treatment (p< 0.05), while the western medicine group TGAb and TMAb value has no obvious change.4) In the aspect of clinical comprehensive curative effect, Chinese and western medicine group effective rate was 93.3%, effective rate of 80% Chinese medicine, western medicine group efficiency is only 53.3%(p< 0.05).Conclusions:This study based on the theory of traditional Chinese medicine, application of Wenyang fuzhengxiaoying Decoction in the treatment of autoimmune thyroiditis (spleen kidney Yang deficiency type). And the use of modern medicine theory and experimental methods, from thyroid hormone (TT4, FT3, TT3, FT4 and TSH), autoantibodies (TGAb and TMAb), clinical symptoms and signs of patients, for a systematic study of 45 patients with AT was carried out for 3 months. Results show that:Wen yang fu zheng xiao ying treatment of Autoimmune Thyroiditis efficacy exact can effectively improve the clinical symptoms, and promote the recovery of thyroid function in patients, reduce thyroid antibody levels; and the same with L-thyroxine can be more effective to improve the thyroid function. And we discussed the mechanism of drug action, to provide the clinical application of the theory and accurate experimental data, in order to enrich the clinical treatment of the disease in china.. |
