Qingwei Jiangni Decoction In Treatment Of Gan Wei Gloomy Heat Type Gastroesophagel Reflux Diseases Clinical Observation And Vasoactive Instestinal Pepide Influence

ObjectiveTo observe the clinical curative effect of Qingwei Jiangni Decoction(QWJNF)on gastroesophagel reflux diseases(GERD)(Gan Wei Gloomy Heat Type) and vasoactive instestinal pepide influence.Methods70 GERD cases with diagnosis standard were randomly divided into treatment group and control group,35 cases per group.Traditional Chinese medicine group were treated with QWJNF,Rabeprazole and Mosapride.The control group were treated with Rabeprazole and Mosapride.Other drugs were discontinued during the treatment. After eight weeks,we observed the comprehensive curative effect and TCM syndrome curative effect,gastroscope signs under mucosa,also the level of VIP.Results1)The treatment group results showed that:15 cases got clinical cure;10 cases were powerfully effective;7 cases were effective,3 were noneffective;the total effective rate was 91.43%.The control group results showed that:10 cases got clinical cure;8cases were powerfully effective;6 cases were effective;11 were noneffective;the total effective rate was 68.57%.Two groups of comparisons difference was statistically significant(P<0.05).2)The comparison of TCM syndromes:(1)The comparison of the combined effects of various symptoms: in the aspects of the main symptoms(heartburning,acid reflux and substernal burning),there were no statistical significant difference between the results of the two groups(P>0.05);in the aspects of the secondary symptoms(the epigastric and abdominal distention,stomach noisy,bitter and dry mouth,irritability,eructation),the treatment group was better than control group(P<0.05);(2)Comparison of the changes of the major and secondary symptoms total scores:two groups in the main symptom total score,secondary symptoms total score and syndrome total score,before and after treatment in the group compared statistically significant differences(P < 0.05);aftertreatment,the total score of main symptoms of the two groups,between groups showed no statistically significant difference(P > 0.05),and in the secondary symptoms total score and total score of symptoms,the comparion of the two groups had statistically significant differences(P < 0.05).(3)To compare the changes of the symptom scores before and after treatment:after treatment in each group were significantly better than those before treatment(P<0.05);after 2 courses of treatment,in improving the degree of acid reflux and heartburn,the effect of the two groups were equivalent(P>0.05),in the other clinical symptoms of remission degree,the treatment group was significantly better than the control group(P<0.05).3)Comparison of the symptom under the gasrtoscope:compared with before treatment the stomach mucosa was obviously improved in both groups,the treatment group in improving the mucous menbrane inflammation was better than that in control group(P<0.05).4)The comparison of the level of VIP in plasma:After treatment,the level of plasma VIP in the treatment group was significantly decreased,compared with the control group,the difference was statistically significant(P<0.05).ConclusionsQingwei Jiangni decoction combined with western medicine could significantly improve Ganwei gloomy heat type gastroesophagel reflux diseases in patients with clinical symptoms and endoscopic esophageal mucosal inflammation degree,but also can reduced the level of plasma VIP.It was worthy of further researching and application in clinic.
