| Objective:The factor analysis is used to validate how well the structure of the competency model for interventional radiology nurses having been built relative to the structure of the competency questionnaire for interventional radiology nurses which has been designed and has highly reliability and validity in this study.The competency model which has been validated can be used as reference standard for guiding nursing work,training nurses and performance appraisal and so on.The competency questionnaire for interventional radiology nurses can be used as evaluation tool for personal development,competing professional title and so on.All of them are advantageous to make good use of interventional radiology nursing human resources and promote the development of interventional radiology nursing.Methods:This study is divided into three parts:Part One: Designing competency questionnaire for interventional radiology nursesThe competency questionnaire for interventional radiology nurses is preliminarily designed based on the competency model and dictionary for interventional radiology nurses having been built.Then,the questionnaire is revised accord to the opinions of panel meeting,item analysis,reliability analysis and the comparison of self-evaluation and peer-evaluation results of pre-research.Part Two: Validating competency model for interventional radiology nursesThe competency questionnaire for interventional radiology nurses is used to survey the competencies of interventional radiology nurses by sampling survey and self-evaluation.837 questionnaires are issued and 740 questionnaires are effective.According to the simple random sampling,the sample is divided into two parts.Half of them are used to determine the factor structure of the questionnaire by exploratory factor analysis,and the other half are used to analysis how well the questionnaire structure fits the clinical data by confirmatory factor analysis.Then,the reliability and validity of simplified questionnaire is analysied.Part Three: Analyzing the present situation of competencies of interventional radiology nurses10 variables including age,gender,marital status,education,professional title,management position,working-life as a nurses,working-life in cath lab,hospital level are investigated to explore if there are significant difference between them.The purpose of this study is to understand the present situation of interventional radiology nurses' competencies and provide reference for the construction,training and development of interventional radiology nursing.Results:Part One: Designing competency questionnaire for interventional radiology nursesThrough two rounds of panel meeting,the experts' opinions are tending to be consistent and a questionnaire including 7 dimensions and 37 iterms is designed.In pre-survey,for the critical ratio of the questionnaire,all of the 37 items are reached significant standard(P < 0.05).The correlation coefficient between the item and the total points ranges from 0.322 to 0.684,so the 37 items of the questionnaire are reserved.But because some of the results ranges from 0.30 to 0.40,the contents of number 1,3,5,8,9,10,12,13,15,19,21,34,37 are modified.The cronbach's a coefficient of the questionnaire is 0.916 and the split-half reliability of the questionnaire is 0.708.The comparisons of self-evaluation and peer-evaluation show that the differences between them are not significant(P>0.05).Part Two: Validating competency model for interventional radiology nursesThe competency questionnaire for interventional radiology nurses is used to survey the competencies of interventional radiology nurses by sampling survey and self-evaluation.837 questionnaires are issued and 740 questionnaires are effective.The structure including 4 dimensions and 30 competencies is obtained by exploratory factor analysis and the rate of total variance explained is 59.425%.Then,confirmatory factor analysis is used to analysis how well the clinical data fits the structure of the competency model.All kinds of fitting indexes meet statistics requirements(X2/df=2.083;IFI=0.931,CFI=0.931,TLI=0.925,NFI=0.876,RMR=0.026,RMSEA=0.054).The final questionnaire is composed of 4 dimensions and 30 iterms,and the four dimensions respectively are interpersonal coordination and control ability,professional knowledge and skills,achievements and security and service.Although the final structure of questionnaire does not agree with the original model,the final structure of questionnaire is more simplified than the original model.The cronbach's a coefficient of four dimensions range from 0.816 to 0.953,and the cronbach's a coefficient of the questionnaire is 0.946(> 0.8).The split-half reliability of four dimensions range from 0.846 to 0.950,and the split-half reliability of the questionnaire is 0.889.And the validity of the questionnaire meets statistics requirements.The correlation coefficients between the item and the total points of dimensions are higher than 0.5.Part Three: Analyzing the present situation of competencies of interventional radiology nursesThrough the statistical analysis of 740 cases,the mean of four dimensions range from 17.07 to 43.72,the subject averages of the four dimensions range from 3.41 to 4.38.It shows that the interventional radiology nurses pay attention to the professional knowledge and skills and service for patients,but the attention to the interpersonal coordination and the research development must be strengthened.In addition,the differences of the four dimensions and the questionnaire in the age,marital status,education,professional title,headcount,working-life as a nurse,working-life in cath labs have statistics significance(P<0.05).The differences of the achievements in gender(P=0.771),the interpersonal coordination and control ability(P=0.074),professional knowledge and skills(P=0.240)in the level of hospital don't have statistics significance.Conclusions:The factor analysis is used to validate how well the structure of the competency model for interventional radiology nurses having been built relative to the structure of the competency questionnaire for interventional radiology nurses and perfect the competency model.Finally,the competency model for interventional radiology nurses which has been validated and has a good fitting effect is composed of 4 dimensions and 30 competency indexes,and the competency questionnaire for interventional radiology nurses which has been designed in this study has highly reliability and validity.The competency model and questionnaire for interventional radiology nurses lay a foundation for clinical application and provide theory and practical basis for the training,performance assessment of interventional radiology nurses. |
