Objective:This study is designed to screen the optimal competency elements of interventional radiology nurse by researching their post competency and to construct a competency model which can distinguish their level of performance, guide the selection of interventional radiology nurse appointments, education and training. The purpose of constructing this model is to provide scientific standards for the training of interventional radiology nurses. For HR, this model can provide an objective standard for personnel selection, performance management and job evaluation. Above all, our study aims to promote optimize allocation of nursing human resources and long-term development of interventional nursing care team. Thereby, this can improve our interventional radiology nurse training system and make the interventional radiology nurse to play its role more suitable for patients. Methods:This study is divided into two parts:Part I: Preparation of interventional radiology nurse competency dictionaryThe preliminary competency dictionary of interventional radiology nurse was constructed by combining literature review and discussion of expert group. Then revise the competency dictionary by using the Delphi method.Part II: Construction of interventional radiology nurse competency modelFirst, performance standards of interventional radiology nurses were determined by expert group meetings. Then 20 interventional radiology nurses were chose from outstanding group and general group to do the interview by behavioral event interview(BEI) method. The interview texts were analyzed by the constructed dictionary. The frequency and level of competency appeared in the text of interviews were analysis statistical. Finally, the differences of competency between outstanding and general group were tested by distinguished method. These parameters were used to construct the competency model of interventional radiology nurse. Results:Part I:After two rounds of expert consultation, 15 assumption features were removed and 24 assumption features were revised. The "Interventional Radiology Nurse Competency Dictionary" was finally constructed and contains 12 competency dimensions and 52 competency elements. Assumption features and their corresponding behavior classification description were included. The mean number of all assumption features is higher than 4.00, the highest was 4.88(innovation capacity), the lowest was 4.00(leadership and management skills); full marks ratio was more than 20%, the highest was 88%(innovation capacity), the lowest was 23%(radiological protection methods and management of excessive irradiation personnel, leadership and management skills, personal qualities); coefficient of variation were less than 0.25, the highest was 0.20(management of DSA common machine malfunction), the lowest was 0.07(innovation capacity). These parameters indicated a consensus of experts, the Kendall coordination coefficient of 26 expert was 0.100(P <0.05).Part II:A total of 19 interview texts were obtained, including 194,848 words and 34 coding competency. The T value of the interview length between outstanding and general group was 0.405(P> 0.05). The average total frequency of competency appearing in outstanding group was 60.50 times(SD = 14.07) and 46.74 times(SD = 8.10) in generally group. The difference test t value was 4.95(P<0.05). Of all 19 interviews, the competency average scores were significantly associated with the length of the interview in one case, the frequency of competency occurrence was significantly associated with the length of the interview in 5 cases, and the highest level of competency was occurred in 8 cases. After analyzing the average score of competencies between two groups, a total of 17 competencies were able to distinguish the difference between the two groups of interventional radiology nurses. Thus, the competency model was constructed containing that 17 competency elements and five dimensions. The five dimensions include personality characteristics(physical quality, sterile consciousness, inner concentration, confidence, sense of responsibility), the ability to provide direct clinical care(the ability to analyze the condition of patients, radiation health knowledge, intervention surgical cooperation), communication and collaboration(verbal and communication skills, teamwork), emergency rescue(control, rescue skills, predictability, flexibility), research and development(innovation, subject interest, professional learning skills). Conclusion:In this study, the competency dictionary was constructed by combing Delphi method, behavioral event interview, expert consultation and panel discussions. By using this dictionary, the records of behavioral event interviews was able to decode and the parameters was obtained to construct the competency model of Interventional radiology nurse ultimately. This model provides a practical significance for guiding the personnel selection, performance management and job evaluation of interventional radiology nurse.
