With the development of society and economy, the change if medical model, rapid flow of city population, especially in recent years, the outbreak of SARS, bird flu, the spread of AIDS, the public health problems that global people are facing has rapidly evolved. When a local area happens epidemic and other emergencies, how to leverage limited resources efficiently, improve the government’s rapid response to emergencies and risk resisting ability. And to provide more efficient emergency services to the public, it has become one of the main content of strengthening city management.This paper uses epidemiology investigation and management system project in one city(hereinafter referred to as the City B) as background, analysis the requirements of epidemiology investigation and management system, design the overall system architect. The system is divided into the data layer, service layer and front layer. By the design of system architect, and implement the key technology as an example, describe the design and implementation of epidemiology investigation and management system integration with forecast and early warning, geographic information system. This system has passed the system test, and has been practical running in city B. The test and application status indicates that this system is workable and valid.Compared with the other same kine of systems, the main research work in this paper has the following characteristics:1. This system has pre-created 37 kinds of case information report including class A, B and C. When some new epidemic disease happen, if need IT specialist to develop new case information report and deploy later, the epidemic control does not respond in a timely manner. This system has implement the dynamic form technology, allowthe system administrator create new disease and the corresponding case information report through the system interface. It has improved the epidemiology investigation service response faster and more efficient.2. The district center of disease control and prevention in City B report influenza like illness number, fever number and diarrhea number every day. This system implement forecast and early warning by using time series analysis. By practical running in city B, it has proved to have positive effect for prevention and early warning the epidemiology. 3. The visual display based on geographic information system(GIS). With residential address, scope of activities, close contact with others information, combined with geographic information system, spatial data, provide the effective information tool and scientific decision support the epidemiological investigation method.
