Objective To assess the diagnostic value of cervical screening technique to high-grade cervical lesions for women in Zunyi city.Methods Data were collected from 13314 cases who voluntarily accepted cervical screening for the first time in the gynaecology clinics and medical examination center of Affiliated Hospital of Zunyi Medical College during August 2014 to December 2015. According to the procedure of cervical three-step-screening technique, the first-stage screening was cervical cytological test(including liquid-based cytology test and cellular DNA quantitative cytology) combined with human papilloma virus(HPV) typing test. Abnormal cases in the stage-one were selected to receive the second-stage screening: vaginoscopy. Similarly, abnormal ones proceeded to be examined in the third-stage with cervical biopsy. Ultimately, the diagnostic efficacy about the first-stage and the second-stage cervical screening was evaluated for high-level cervical lesions based on different screening methods.Ruslt 1. Compared with three kinds of cervical screening methods in the first-stage and the second-stage, the positive rate of vaginoscopy was the highest(56.89%)(P<0.05). The pathological detection rate of high level of cervical lesions didn’t show statistical significance among four cervical screening methods(P>0.05). 2. The pathological detection rate of high-grade of cervical lesions based on combination of three methods was higher than that in combination of two methods(P<0.05). Compared with the joint detection of liquid-based cytology test, detection of cellular DNA and typing of HPV, combination of four methods couldn’t enhance the pathological detection rate(P>0.05). 3. The infection rate of women with high risk HPV in high-grade of cervical lesions in Zunyi city was 87.20%. The major types of high risk HPV were 16, 58, 52, 33 and 18(P<0.05). The type 41 of HPV wasn’t detected in all cases. 4. In the first-stage and the second-stage cervical screening, the sensitivity(94.92%) and specificity(73.17%) were the highest by using the joint detection of liquid-based cytology test(LCT), detection of cellular DNA and typing of HPV. 5. The pathological detection rate of high level of cervical lesions in 40-49 years old group of women was higher than that in other age groups(P<0.05).Conclusion 1. It is suggested that the joint detection of LCT, detection of cellular DNA and typing of HPV may be the prefer solution of stage-one cervical screening for women in Zunyi city. 2. More attention should pay on controlling and preventing the infection of these high-risk types of HPV which include 16, 58,52, 33 and 18 in Zunyi city. 3. Standardized cervical screening should be focus on 40-49 years old women in Zunyi city.
