Objective:Cervical intraepithelial neoplasia is precancerous cervical cancer.Timely detection of early lesions of cervical cancer treatment and prognosis of critical importance.Follow-up investigation by our hospital cervical lesions in patients with liquid-based cytology and cell DNA screening using quantitative techniques.To assess and investigate the clinnical application value of DNA quantitative cytology combined with cervical liquid-based cytology in cervical intraepithelial neoplasia.Finding suitable methods in screening of cervical lesion.Methods:515 cases of patients were enrolled in this study,all of whom were screened by DNA quantitative cytology with cervical liquid-based cytology in outpaint of Department of Gynecology of Yan’an University Affiliated hospital from January 2015 to September2015.101 patients showed positive results of cervical liquid-based cytology and/or DNA quantitative cytology, and were given colposcopy and histopathological examination,with histopathological examination as the diagnostic standard. All data were input into SPSS21.0 software for statistical analysis anda lculated the sensitivity and specificity of screening methodscount data, use rate or the constituent ratio to represent.The difference was statistically significant when P <0.05.Results:1.In TCT detection,agreement rate of CINII+was40.00%(28/70); In DNA quantitative cytology, agreement rate of CINII+ was 43.67%(38/87), and The two combined was 68.75%(33/48). The combined screening is better than single screening.2.In TCT detection,with≥ASCUS as the standard for biopsy,the sensitivity of CINⅡ+ was 71.79%.In DNA quantitative cytology, with more than or equal to threeaneuploid cells as biopsy criteria, the sensitivityof CINⅡ+ was 82.05%.There were statistically significant differences between the two methods.3.With TCT≥ASCUS and ≥3 heterploid cells in DNA quantitative cytology as the standard for biopsy, the specificity, positive predictive value of CINⅡ+ was84.61%,75.80%, 68.75%. In TCT detection,the sensitivity,specificity, positive predictive value of CINⅡ+ was 71.79%,32.25%, 40.00%.The two combined with sensitivity, specificity, positive predictive value is better than that of TCT.4.In DNA quantitative cytology,with 1-2 aneuploid cells as the stangard of biopsy,the sensitivity and specificity of the CINI+ was 94.64%,24.44%;with≥3 heteroploid cells as the standard for biopsy,the sensitivity and specificity of CINI+was 75%, 93.33%.The sensitivity of two kinds was statistically significant.5.In DNA quantitative cytology, positive detection rate of CINII+ with1-2heteroploid cells as the standard for biopsy was 14.29%, while with ≥3 heteroploid cells as the standard for biopsy was 71.11%, and there were statistically significant differences.Conclusion:1.The combined screening with agreement rate is better than single screening.2.With the increase of DNA heteroploid cells, detection rate of high level cervical lesion is increased.3.DNA quantitative cytology has higher sensitivity than TCT in screening high level cervical lesions.4.The two combined with sensitivity, specificity, positive predictive value is better than that of TCT. Both DNA quantitative cytology and cervical liquid-based can significantly improve the screening efficiency.
