Objective: To study the application value about Cervical Liquid-based Cytologyand HPV testing and DNA quantitative cytology in the Diagnosis of cervical lesions.Methods:2156patients were detected by cervical liquid-based cytology and HPVtesting and DNA quantitative cytology test, and in which of221cases with colposcopyand cervical biopsy were retrospectively analyzed. The histological results as the goldstandard,Analysis the value of TCT HPV, DNA quantitative cytology alone orcombination in cervical lesions.Results:1. Analysis HPV results:In the CINⅠ-Ⅱ grade,26-30years is the peakincidence.In the CINⅢ grade-CC,31-40years and>45years are the two peakincidences. There are single or multiple high-risk HPV infection. The sequence isHPV16(22.29%)、HPV58(17.71%)、HPV33(9.71%)、HPV52(7.43%)、HPV53(5.71%)、HPV18(5.14%) in the all HPV types. HPV6(5.14%) andHPV11(5.14%) is the most common type in the low-risk HPV.2. The sensitivity,specificity,positive predictive value,negative predictive value of TCT and HPV andDNA quantitative analysis alone were69.77%and79.07%and77.52%,38.04%and63.04%and48.91%,61.22%and75.00%and63.69%,47.30%and68.24%and60.81%respectively.3. The specificity of TCT in comparison with HPV detection method wasstatistical significance(P<0.01);the sensitivity was no statistical significance(P>0.05).The sensitivity and specificity of TCT in comparison with DNA quantitative analysisdetection methods were no statistical significance(P>0.05). The sensitivity andspecificity of HPV in comparison with DNA quantitative analysis detection methodwere no statistical significance(P>0.05).4. The sensitivity,specificity,positivepredictive value,negative predictive value of TCT combined with HPV detectionmethods were85.27%,77.17%,83.97%and78.98%.The sensitivity and specificity ofTCT in comparison with TCT combined with HPV detection methods were statistical significance(P<0.01). The specificity of HPV in comparison with HPV combined withTCT detection methods was statistical significance(P<0.05);The sensitivity was nostatistical significanc(eP>0.05).5. The sensitivity,specificity,positive predictive value,negative predictive value of TCT combined with DNA quantitative analysis detectionmethods were89.15%,84.09%,86.47%and84.09%. The sensitivity and specificity ofTCT in comparison with TCT combined with DNA quantitative analysis detectionmethods were statistical significance(P<0.01). The specificity of DNA quantitativeanalysis in comparison with DNA quantitative analysis combined with TCT quantitativeanalysis detection methods was no statistical significance(P>0.05);the sensitivity wasstatistical significance(P<0.01).6. The sensitivity and specificity of TCT combinedwith HPV in comparison with TCT combined with DNA quantitative analysis detectionmethods were no statistical significance(P>0.05).5. The detection rate of ASCUS withcervical lesion was56.25%. HPV as a shunting method for ASCUS:The detection rateof the HPV positive group was74.42%,The detection rate of the HPV negative groupwas35.14%. A comparison of two groups was statistical significance(P<0.01).Contrasting the detection rate of HPV positive group and ASCUS with noshunting, there was statistical significance(P<0.05). DNA quantitative analysis as ashunting method for ASCUS:The detection rate of the positive group was74.00%,Thedetection rate of the negative group was26.67%. A comparison of two groups wasstatistical significance(P<0.01). Contrasting the detection rate of DNA ploidypositive group and ASCUS with no shunting, there was statistical significanc(eP<0.05).There are no statistical significance between the two groups (P>0.05).Conclusion:1.The infection of HPV is age distribution. All levels of cervicalLesions are main infected with high risk HPV genotypes. The infection rate of HPVshowing a rising trend as the grade of cervical lesions increasing.The HPV16is thehightest infected type of all HPV types, showing a rising trend as the grade of cervicallesions increasing.2. The sensitivity and specificity of HPV is superior to the TCTdetection method. No significant difference between HPV and DNA quantitativecytology detection method. No significant difference between TCT and DNAquantitative cytology detection method.3. TCT combined with HPV or DNAquantitative cytology can improve the sensitivity and specificity, It is better than thesingle application of TCT or HPV or DNA quantitative cytology detection method.4. HPV or DNA quantitative cytology can improve the detection rate,it is a effectiveshunting method for the patient of ASCUS.
