Objective The study was to identify the influence of Xiao Ding ointment on fracture union by experimental study and clinical observation.Methods 64 healthy New Zealand white rabbits Were randomly divided into two groups.One group was the experimental group,Another group was the control group.every group had 32 rabbits.The 3mm-long bone defect were made in both tibia of each rabbit.After the success of modeling,the experimental group was treated by Xiao Ding ointment external application.The control group not treated. After 1,2,4,6 weeks of postoperation,collected blood from heart and sacrifice the experimental animal respectively.Then,assess the fracture healing through X—ray and HE,Masson staining.Determinate the serum osteocalcin concentration by using the method of ELISA.Observe the changes of gene and protein expression of BMP-2,BMP-7,TGF- β1 through immunohistochemistry and RT-PCR in the course of treatment.Totally 97 Colles fracture patients selected standardly were randomized into two groups, the experimental group were 48 patients,the control group were 49 patients.All of them were reduced early and fixed with small splints in the neutral position,while patients in the treatment group were pasted Xiao Ding ointment on the wound.The dressing was changed two times a week,for 6 weeks.After the end of treatment 6 month,in accordance with the "standards of TCM syndrome diagnosis and treatment",the standard of fracture curative effect of lower end of radius,assess the patient’s clinical efficacy.The pain and swelling severity were compared with the two groups by integral method.Results X-ray films showed that the experimental group’s callus appeared earlier and had more callus than the control group.Fracture healing time was earlier than the control group.Tissue sections,HE and Masson staining showed that the experimental group’s fiber connection was earlier than the control group. Osteoblasts, osteoclasts, callus had more than the control group.Fracture healing time was earlier than the control group.The concentration of experimental group’s Serum Osteocalcin was higher than the control group at each time point(P<0.01).The two groups had peak concentration in 2-4 weeks of postoperation.The experimental group’s BMP-2, BMP-7,TGF-β1 MOD values of immunohistochemistry and gene expressive quantity of RT-PCR were higher than the control group at each time point(P<0.01).The experimental group’s BMP-2, BMP-7, TGF-β1 MOD value of immunohistochemistry and gene expressive of RT-PCR appeared peak in about 2 weeks of postoperation,but the control group appeared late.The clinical effect of patients in the treatment group were better than that in the control group.The cure rate of treatment group was higher than that of the control group(P<0.05).3days and 7 days after manual reduction,the differences of pain and swelling scores by VAS in the treatment group were better than that in the control group(P<0.01).Conclusion Xiao Ding ointment can improve the serum osteocalcin concentration and express the BMP-2, BMP-7, TGF- β1 gene and protein of callus peak values early and increased so as to promote fracture healing.
