| With the intensified conditions of aging population, the demand of the elderly for pension service and medical service is growing. Under the circumstances of economy conditions, production level and traditional idea of pension service the home-based healthcare for the elderly lay a foundation for social pension service system. As home-based pension service without medical service cannot meet the pension-service demand of the aged, developing home-based healthcare for the elderly with medical service is beneficial for positive and healthy aging phenomena.This research analyzed and demonstrated the necessity and feasibility of integration of home-based medical service and pension service in the main urban area of ChongQing city. Though stratified randomized sampling Yuzhong district, Shapingba district and Yubei district were selected and 444 questionnaires about the demand of healthcare and pension service in the main urban area of Chongqing city were effectively collected. Logistic analysis for financial protection, medical need, life care and spiritual consolation was executed, then results manifested: in terms of economic need, the aged who has a lower income and choose the home-based health care for the elderly or are unhealthy are more young, have more economic need; in medical need respect, the elderly that choose better hospital when going to see a doctor and were not satisfied with the conditions of keeping health and pension service had more needs for medical service; in life care respect, the aged who lived with relatives or lived solitarily needed more life care; in the respect of spiritual consolation, the elderly who had a bad self-care or had an unharmonious relationship with families had more demands for spiritual consolation. The theoretical research was executed from the viewpoint of the governmental responsibility for medical service and pension service, which summarized the current conditions in the respect of main body of supplies, subject, content, methods and supporting measures. By interviews and on-site survey investigating the dilemma how home-based health care for the elderly fitted into the integration mode of pension service and medical service was implemented. At last, this paper raised main strategies: strengthening governmental responsibility, broadening financing channels, establishing the coordination mechanism of need orientation and gradually piloting and promoting.This paper enriched the research on integration of medical service and pension service, refining the concept “integration of home-based service and pension service” and deepening the study on the basis of research topic and further surmounting the limit of home-based medical service and pension service concentrated in the nursing institution for the aged. Though integrating resources of pension service and grassroots health service the quality of home-based pension service and medical service for the elderly could be guaranteed and the best way meeting medical service and pension service under the conditions of traditional value conception and socio-economic situation would be digged out. |
