Research On Price Relationship Of Medical Service Items

BackgroundFor a long time,the price of medical services in China has been distorted,most of the medical services are below the cost,and there are unreasonable price relations among regions,hospitals and project systems.In order to solve a series of problems in the field of medicine,China began the road of medical reform in the early 1950s.2009 is the first year of the new round of deepening the reform of the medical and health system.The new medical reform clearly proposed that the price management of medical services should be standardized and the compensation mechanism of public hospitals should be reformed in the reform of the medical price,thus the comprehensive reform of the medical price officially began.In June 2012,China began to implement the reform of county-level public hospitals in an all-round way,and gradually carried out the reform of canceling drug addition and consumables addition.In the top-level design of the government,it proposed to gradually rationalize the price-ratio relationship and change the compensation mechanism of public hospitals from drug addition,medical service income,government compensation to medical service income and government compensation.Since then,the price adjustment of medical services has been achieved.Substantial progress.Perfecting the price system of medical services and straightening out the price-ratio relationship of medical services have become the core and key to deepen the reform of medical and health system.In terms of the formation mechanism of medical service price,the top-level design of the government proposes to establish a dynamic price adjustment mechanism based on the change of cost and income structure,whose essence is to dynamically adjust the price of medical service based on project cost.At present,most hospitals have not yet carried out full cost accounting,which makes the price adjustment have structural defects and has entered the attack in deepening the reform of public hospitals.In the tough period,it is urgent to carry out relevant research on the price comparison of medical service projects between different provinces of the same project,and analyze the compensation degree of price to cost based on scientific and reasonable estimation of project cost,to fully grasp the current situation of the price system of medical service projects,so as to provide decision support for dynamic adjustment of medical service price.ObjectiveThis study is based on the current norms and price lists of medical services in eight provinces,namely,Guangdong,Jiangsu,Zhejiang,Hubei,Hunan,Fujian,Liaoning and Shanxi,to analyze the regional price-ratio relationship between the medical services price of the same project.At the same time,based on the cost estimation of color Doppler ultrasound examination project in a third-class A hospital in Guangzhou,the cost of color Doppler ultrasound medical service project is analyzed empirically to measure the compensation degree of price to cost,in order to provide relevant policy suggestions for rationally determining the price adjustment space,further refining the price adjustment scheme,and promoting the establishment of dynamic adjustment mechanism of medical service price.MethodsThis paper takes the comparative price research of medical service items as the main line,and comprehensively uses various research methods,such as literature research method,comparative analysis method,statistical analysis method,field investigation and research method,to analyze the regional differences in price of medical service items and the relationship between cost and price.Results(1)Uneven prices of medical services in provinces and cities.Overall,the prices of medical service items in eight provinces are mainly concentrated between 0-200 yuan,the prices of clinical diagnosis and treatment medical service items are relatively high,and the prices of comprehensive medical service items are relatively low.Further analysis of the prices of various types of main projects revealed that the regional differences in the prices of medical services were large.(2)Price compensation for color ultrasonic medical services is basically in place.The results of this study show that the price of medical services such as color Doppler ultrasound examination is basically in place to compensate the cost of the project,indicating that the income from large medical equipment inspection projects is one of the important ways for hospitals to compensate.At the same time,the project cost of different parts under the same medical service project also has a large difference,therefore,when pricing the medical service project,we can consider grading pricing according to the inspection site.(3)The name and code of medical service expansion project are not unified yet.Through the analysis of the medical service projects in each province,it is found that the standard versions of medical service projects in each province vary greatly,which is manifested in three aspects:first,the standard of medical service projects in each province is not unified;second,the coding principle of medical service projects in each province is different;third,the standard versions of medical service projects in each province are different.
