ObjectiveTo explore improved method of incomplete unilateral cleft lip in order to achieve more cosmetic operative effect.MethodsFifty-nine patients with unilateral incomplete cleft lip were randomly selected in the hospital from January 2014 to May 2015, and the modified method was performed. Fixing surgery point and incision line of lip skin is designed according to the contralateral philtrum ridge in order to get the postoperative natural philtrum structure. The triangular flap of affected side nasal base is inserted into triangular defect besides the suffered columella nasi to lower the upper lip. Nostrils width is adjusted by the length of triangular flap. The orbicularis oris muscle was dissected and anchored on the septal suture, then reconstructed by mattress-suture. Incision of Changgeng’s method is applyed to reset the skin, mucosa and cartilage of affected nasal. The effect was measured exactly after operation for objective evaluation.Fifty patients aged from 19 days to 9.7 years were treated with this modified method. All the patients presented natural philtrum structure. The scars were at the philtrum ridge; the labial arch was in good shape and the red lips were well-stacked. Regularity of red labial margin was showed.Continual orbicularis oris muscle was observed when lip exercising.Thirty-eight cases of primary nasal repair were successful; both nostrils were in symmetry. Moderate corrective effects were observed in 12 cases.Affected nostril showed a little lower and flatter than another side. Ala nasi of 9 cases without wearing nasal model was collapsed unexpectedly after 2months.ConclusionWith this modified method of incomplete unilateral cleft lip, the postoperative result can more approach nature natural lip nasal structure.
