Objective:To compare the training needs self assessment of pediatric nurses in different levels and his assessment of head nurses, based on clinical ladder program, through the self serving survey questionnaire on training needs of inservice pediatric nurses.To understand the difference between the two, to provide reference for nursing managers to develop standardized training program for specialist nurses.Methods:(1) The design of the questionnaire: Consulted the relevant literature at home and abroad, according to the research purpose, interviewed nurse. so as to design the first draft. The reliability and validity of the questionnaire were tested.Ultimately, a survey questionnaire was formed.(2) Questionnaire survey: convenient and cluster sampling method were adopted. Conveniently selected the Shanxi province two three level of first-class hospital. The training needs of inservice pediatric nurses were investigated by the self-made questionnaire. Training needs for pediatric N0~N4 nurses in a hospital were self assessmented. The hospital pediatric head nurse needed to carry on his evaluation to the hospital N0~N4 grade department of pediatrics nurse.The training needs of pediatric nurses in different levels and head nurses’ training expectation for each level were compareed. The same method was used to the second hospital.Results:(1) Questionnaire reliability and validity:The questionnaire test-retest reliability was 0.878.Cronbach’s α value was0.985.Split half reliability value was 0.845.The content validity index was 0.862.13 common factor was extracted.The roots of each common factor values were greater than 1. The cumulative contribution rate was 77.29%. Each item on a factorloading was greater than 0.4.(2) The difference in A hospital between each level pediatric nurses’ training needs and head nurses’ expectations was statistically significant, including:(1)N0 level nurses needs wre different from head nurses’ expectations(P<0.05)including nursing techniques, specialized nursing technique of children, nursing education, nursing research, safety management and the Humanities. The nurse training needs were higher than head nurses’ expectations in nursing education,nursing research(3.90 + 0.78 VS 3.19 + 1.42, 3.76 + 0.86 VS 3.05 + 1.41).The nurse training needs were lower than head nurses’ expectations in including nursing techniques, specialized nursing technique of children, safety management, the Humanities(4.06±0.79VS4.75±0.45, 3.91±0.74VS4.52±0.81, 4.26±0.77VS4.75±0.48,3.83±0.68VS4.58±0.45).(2)N1 level nurses needs were different from head nurses’ expectations(P <0.05) including laws and regulations, basic nursing theory of pediatrics, knowledge and application of drugs in pediatric nursing, nutrition and psychological nursing,basic nursing technology, specialized nursing technique of children, use of medical devices, critical skills,nursing documents, safety management, the humanities and total demand. Moreover, the nurse training demand were lower than head nurses’ expectations.(3)N2 level nurses needs were different from head nurses’ expectations(P <0.05) including nutrition and psychological nursing,critical skills, nursing teaching,nursing management, security management, Humanities and demand total.Moreover, the nurse training needs were lower than head nurses’ expectations.(4)N3 level nurses needs were different from head nurses’ expectations(P <0.05) including basic nursing skills, specialized nursing technique of children,nursing teaching, nursing research and nursing management. The nurse training needs were higher than head nurses’ expectations in basic nursing skills and specialized nursing technique of children(3.68±0.91VS3.24±0.71,3.39±1.01VS2.78±0.98). The nurse training needs were lower than head nurses’ expectations in nursing teaching, nursing research and nursing management(4.12±0.77VS4.69±0.50, 4.15±0.73VS4.69±0.48, 3.89±0.87VS4.56±0.47).(5)N4 level nurses needs were different from head nurses’ expectations(P <0.05) including basic nursing skills,specialized nursing technique of children, use of medical devices, critical skills,nursing documents,nursing management and total demand. Pediatric nurse training needs were higher than the head nurses’ expectations in use of medical devices, critical skills, nursing documents and total demand(4.07±0.92 VS 3.03±0.94, 4.43±0.65 VS3.75±0.96, 4.24±0.88 VS 3.48±1.00,4.07±0.60VS3.65±0.50). The nurse training needs was lower than head nurses’ expectations in nursing management(4.23±0.51VS4.61±0.61).(3) The difference in B hospital between each level pediatric nurses’ training needs and head nurses’ expectations was statistically significant, including:(1)N0 level nurses needs were different from head nurses’ expectations(P <0.05) including laws and regulations,basic nursing theory of pediatrics, the humanities. Moreover, the nurse training needs were lower than head nurses’ expectations.(2)N1 level nurses needs were different from head nurses’ expectations(P <0.05) including laws and regulations,knowledge and application of drugs in pediatric nursing, basic nursing technology, specialized nursing technique of children, use of medical devices, the critical skills. Moreover, the nurse training needs were lower than head nurses’ expectations.(3)N2 level nurses needs was different from head nurses’ expectations(P<0.05)including basic nursing techniques. Moreover, the nurse training need was lower than head nurses’ expectations.(4)N3 level nurses needs was different from head nurses’ expectations(P<0.05)including critical skills. Moreover, the nurse training need was lower than head nurses’ expectations.(5)N4 level nurses needs were different from head nurses’ expectations(P <0.05) including Pediatric basic nursing theory, use of medical devices, nursing research, nursing management, security management and the Humanities. The nurse training needs were higher than head nurses’ expectations in Pediatric basic nursing theory and use of medical devices(4.09±0.70VS3.20±0.85, 4.33±0.57VS3.60±0.55). The nurse training needs were lower than head nurses’ expectations in nursing research, nursing management, security management and the Humanities(3.74±0.97VS4.80±0.22, 4.00±0.87VS4.87±0.19, 4.20±0.85VS4.92±0.18, 4.00±0.75VS4.77±0.25).(4) Common dimensions of differences between nurses and head nurses in two hospitalsThe two hospitals, the N0 level nurses need was lower than head nurses’ expectations in the humanities(P<0.001, P<0.001). The N1 level nurse training needs were lower than head nurses’ expectations in laws and regulations(P<0.05, P<0.05), knowledge and application of drugs in pediatric nursing(P<0.05, P<0.05),basic nursing technology(P < 0.001, P=0.001), specialized nursing technique of children(P<0.05, P<0.001), use of medical devices(P<0.05, P<0.05)and the critical skills(P<0.05, P<0.05). The N4 level nurses need was higher than head nurses’ expectations in use of medical devices(P<0.001, P<0.05). The N4 level nurses need was higher than head nurses’ expectations in nursing management.Conclusions:(1) There are different dimensions in concept of training needs between each level pediatric nurses’ training needs and head nurses’ expectations.(2) Two hospitals of N0 in the humanities, N1 level in the laws and regulations, pediatric nursing knowledge and drug application, Basic nursing skills,specialized nursing technique of children, use of medical devices,critical skills, N4 level nurses’ demand is different from the head nurses’ expectations both in using of medical equipment and nursing management.(3) Between the two hospitals, the same level of pediatric nurses’ training needs and head nurses’ expectations are not the same in different dimensions, the hospital should make targeted hierarchical training program based on their actual situation.
