BackgroundWith the rapid development of medical science and technology,the technical requirements for the nursing profession are getting higher and higher,and the clinical nursing work is increasingly developing in the direction of specialization.Due to the late start of the training of our country's specialist nurses,there is a lack of relatively uniform standards and training models.The current training of multiple forms,multi-category goals,and multiple standards makes the training of specialized nurses insufficient and of low quality,and can not meet the development of clinical nursing.Needs.Clinical research shows that the training of newly recruited nurses is the basis and prerequisite for the training of specialist nurses.With the support of national policies,the training of newly recruited nurses has been universally implemented in all three-level general hospitals in the country.Therefore,on this basis,it is necessary to explore as soon as possible to adapt to the social needs of the professional nurse training model to form a relatively complete post-graduation education system for nurses,improve the quality of clinical nursing services,to deepen the medical reform,to promote nursing personnel training and nursing specialist development is of great significance.The “2+1” specialty nurse training model to be constructed in this study refers to the training model for one-year training based on the two-year new entry training to enhance the clinical specialist nursing knowledge and technical expertise.ObjectiveUnderstand the current situation of nurses training new nurses and the training needs of specialist nurses,and explore the "2+1" training model for specialist nurses that suits China's national conditions.This will provide a theoretical basis for the standardized and systematic construction of professional nurses.MethodsThe questionnaire survey method was used to systematically analyze the current status of new nurses and the training needs of specialist nurses.On this basis,semi-structured interviews and the Delphi method were used to focus on the training objectives and the training process and evaluation of the nurses were included in the target.In the process of implementation,on the basis of safeguard measures,a "2+1" training model for specialist nurses was established.1.Questionnaire method: By reviewing the relevant literature,based on a thorough review and analysis of training research for domestic and foreign specialist nurses,and through pre-surveys,expert content validity analysis and other aspects of control,we have designed and formed a survey of "new entry nurses training status" and "specialized nurse training needs." The questionnaire used the cluster sampling method to collect new nurses,teaching teachers,clinical nurses,nursing managers,and clinicians in 36 tertiary comprehensive hospitals that meet the requirements of the country as survey subjects,among which,The status of new nurses Questionnaire survey: 1,200 newly recruited nurses and teaching teachers,totaling 2400;Questionnaire survey on training needs of specialist nurses,selecting 1,440 clinical nurses,720 nursing managers,and 960 clinicians,totaling 3,120 people.The statistical description of the data was performed by using frequency and percentage;the data were statistically inferred using one-way analysis of variance and correlation analysis,the current status of the new nurses and the training needs of the nurses were analyzed.2.Semi-structured interviewing method: On the basis of the results of the previous survey on “training needs of specialist nurses”,combined with literature research,the Semi-Structured Interviewing Outline of the Training Model for Specialized Nursing Nurses was designed.Target sampling method was used to select semi-structured interviews with representative,authoritative,and extensive experts.When the data was saturated to analyze the surveyed data,there were no new topic keywords.The total was 7 experts.Through Colaizzi data analysis method,the subject of interview subjects was analyzed,and the framework of “2+1” specialist nurse training mode was initially established.3.Delphi Method: On the basis of preliminary discussion of the “2+1” training model for professional nurses,the Delphi Expert Consultation Questionnaire was formed on the basis of group discussion and expert pre-testing.According to the research purpose,21 experts were selected from the aspects of prediction accuracy,authority,and effective response.Through two rounds of Delphi expert consultations with 21 experts,the degree of expert initiative,expert authority,coordination of expert opinions,and consistency of expert opinions were calculated,and the “2+1” specialist nurse training model was further demonstrated.Result1.Questionnaire survey on the status of newly-inducted nurses shows that 91% of teachers believe that the training of new nurses is conducive to the development of specialist nurses,which is the basis and prerequisite for the training of specialist nurses.2.The results of the survey on the training needs of specialist nurses showed that 99.2% of clinical nurses,100% of care managers,and 94.9% of clinicians supported the idea of training specialist nurses.3.The results of semi-structured interviews showed that the 7 experts interviewed included high professional titles(100%),undergraduates(14.4%),masters(42.8%),and Ph.D.(42.8%),hospital management experts(14.4%),and nursing education experts.(42.8%),clinical nurse specialist(42.8%).Aiming at the status quo of new nurses training and training needs of specialist nurses,the theoretical framework for the training model of “2+1” specialist nurses was initially established,and it contained four basic elements: training objectives,training process,assessment evaluation and safeguard measures.4.The two rounds of the Delphi Expert Consultation Questionnaire collection rate were 100%,the expert recommendation rate was 90.5% and 95.2% respectively,and the expert authority coefficient was 0.88.The two rounds of consultation questionnaire result experts have four first-level indicators for the training model of “2+1” specialist nurses,including training objectives,training process,assessment evaluation and safeguard measures;18 secondary indicators,which mainly include theoretical knowledge and training targets.,appraisal content,etc.;58 three-level indicators,including the initial mastery of relevant basic theoretical knowledge,mastery of related professional nursing theory and other knowledge reached a consensus opinion.ConclusionsThis study constructed a “2+1” training model for specialist nurses including four objectives: training objectives,training process,assessment and evaluation,and training support for specialist nurses.This model is expected to provide the standardized nurse training and enrichment of specialist nursing personnel training system.reference.
