Objective:To explore the related risk factors of pulmonary infection of patients after esophagectomy and its preventive measures.Methods:Clinical data of 103 patients undergoing esophagectomy in The 2nd Affiliated Hospital of Kunming Medical University between September 2010 and September 2015 were retrospectively analyzed. According to whether they had pulmonary infection after esophagectomy, all the 103 patients were divided into 2 groups. The 2 groups are the infection group and the uninfection group. Age, sex, smoking history,other underlying diseases,pulmonary function,operation method, operation duration,intraoperative blood loss,anastomosis position,laryngeal recurrent nerve injury,pathological staging,gastroesophageal reflux were compared in the incidence rate of pulmonary infection between the 2 group. Multivariate logistic regression was performed to analyze risk factors of pneumonia after esophagectomy.Results:Single related factors analysis showed old age(≥60 years),long history of heavy smoking(≥ 400 cigarettes per year),DM, FEV1/FVC< 70%,operation time longer than 4 hours, intraoperative blood loss more than 800 ml,inosculation on the arch aorta, laryngeal recurrent nerve injury, gastroesophageal reflux were easier to occur pulmonary infection.The differences were statistically significant(P< 0.05). Multivariate logistic regression analysis showed that old age(≥60 years),long history of heavy smoking(≥ 400 cigarettes per year),DM, FEV1/FVC< 70%,operation time longer than 4 hours, intraoperative blood loss more than 800 ml,laryngeal recurrent nerve injury, gastroesophageal reflux were main risk factors of pulmonary infection after esophagectomy (P< 0.05)Conclusions:Postoperation complicating pulmonary infection after esophagectomy has related to many factors. Strengthing perioperative period management is helpful to prevent pneumonia after esophagectomy.
