BackgroundEsophageal cancer is one of the most common malignant tumors in China. The clinical effect is quite limited, the rate of survival in 5 years is less than 15%. The best way to improve the esophageal carcinoma survival rates is early diagnosis, and the surgical treatment is the most effective method for the esophageal carcinoma. But as a result of patients’state and co-morbid conditions constitution factors, a series of complications are prone to happened after esophagectomy, and the incidence rate of pulmonary infection is in a higher level among them. It seriously affects the patients postoperative recovery and increases the postoperative mortality.ObjectiveTo discuss the risk factors of pulmonary infection after esophagectomy, explore the effective ways to prevent postoperative pulmonary infection, reduce the incidence of pulmonary infection after esophagectomy.MethodsCollected 187 cases of patients who were diagnosed to esophageal carcinoma (Squamous cell carcinomas) at the Thoracic surgery of the first affiliated hospital of XinXiang Medical College from March 2012 to December 2013, all the patients were taken the radical resection of esophageal carcinoma in our hospital.Collecting patients’ clinical datas, such as age, medicalhistory includingchronic obstructive pulmonary disease(COPD), hypertension, diabetes, coronary heart disease, smoking history, surgical method, ventilation mode,the duration of the operation and tumor stage. According to the pulmonary infection happened or not,187 patients were divided into two groups with postoperative clinical observation. The patients with postoperative pulmonary infection were in group one, while the of patients without postoperative pulmonary infection were in another group.Descriptive statistical analysis method was applied to describe the basic data. Chi-square test was used to count data of two variables analysis or T test for measurement data analysis of two variables, compared two groups of patients basic information,The screening effect with the method of logistic regression analysis correlative risk factors of pulmonary infection after esophagectomy.ResultsThrough statistical analysis comparison, the patients with pulmonary infection and without pulmonary infection group’s age, the COPD, diabetes, smoking history, the duration of the operation, tumor stage, and all of them have statistical significance(.P<0.05). Intraoperative ventilation way, surgical procedure, history of hypertension and the combining of coronary heart disease do not produce significant differences (P>0.05).ConclusionThis study found that patient age, smoking history, chronic obstructive pulmonary disease, an diabetes mellitus, the duration of the operation more than3 hours and Tumor stage in phase Ⅲ+Ⅳ are risk factors of postoperative pulmonary infection.
