Objective: To compare the rate of postoperative pulmonary infection of the twoapproaches of thoracotomy in the lower segment esophageal cancer,the one through leftchest entrance, the other through right chest.And to explore the cause of pulmonaryinfection after esophageal resection and put forward the prevention measures.Methods: aretrospective analysis was proformed of482cases of esophageal carcinoma,which wereretrieved from the Xinjiang Uygur Autonomous Region people’s Hospital and the SecondAffiliated Hospital of Xinjiang Medical University between2003January to December2012, according to different operation approach, the cases were divided into the left chestgroup (the left chest posterolateral single incision approach) including240cases,and theright chest group (via right chest posterolateral abdominal median incision of242casestwo).to statistic the incidence of postoperative pulmonary infection and compare theirdifferences in preoperative, intraoperative, postoperative factors, analysis of esophagealcancer after operation period of the main risk factors of pulmonary infection.Results: ofthe482patients were successfully completed the operation, postoperative pulmonaryinfection56cases (11.6%),25cases in the left chest group (10.4%),31cases in the rightchest group (12.8%),the rate of the postoperative pulmonary infection between two groupswas not stastistically significant(p>0.05). Postoperative pulmonary infection have notrelative to sex,nationl,and the approach of anastomosis. risk factors included: age,preoperative long history of smoking and poor lung function, longer operation time,intraoperative anastomosis on the bow and the recurrent laryngeal nerve injury,postoperative gastroesophageal reflux and non epidural analgesia.Conclusion:compare therate of pulmonary infection of the two group, there was no stastistically significant (P>0.05), postoperation complicating pulmonary infection has related to many factors,perioperation period management, is the key factor to the prevention of postoperativepulmonary infection.
