ObjectiveThe purpose of this analysis is to assess outcomes of early ART in adult patients with HIV who are eligible for treatment, to understand the factors for virological failure, retention rate and mortality, and to identify ways to improve HIV early treatment outcomes and put forward suggestions for policy implementation in China.MethodsWe did a nationwide retrospective observational cohort study with data from the 95% CI 1.05-1.34) and Injection drug use (adjusted odds ratio [OR] 2.63,95% CI 2.31-2.99)&(adjusted odds ratio [OR] 2.87,95% CI 2.47-3.34) are more likely to fail in virological suppression.3. Patients with transmission route of Heterosexual (adjusted odds ratio [OR] 1.51,95% CI 1.39-1.65)&(adjusted odds ratio [OR] 1.65,95% CI 1.48-1.85) and injection drug use (adjusted odds ratio [OR] 3.87,95% CI 3.50-4.28))&(adjusted odds ratio [OR] 4.30,95% CI 3.79-4.88) have lower retention rate, compared with MSM. Participants with age≥60 are also tend to lose to follow-up or stop treatment(adjusted odds ratio [OR] 1.60,95% CI 1.14-1.77) & (adjusted odds ratio [OR] 1.68,95% CI 1.47-1.91).4. Mortality of female (adjusted odds ratio [OR] 0.72,95% CI 0.61-0.85) is lower than male. Heterosexual (adjusted odds ratio [OR]1.51,95% CI 1.23-1.87) and injection drug use (adjusted odds ratio [OR] 6.23,95% CI 4.97-7.81) are risk factors. An older age is also risk factor (adjusted odds ratio [OR] 6.91,95% CI 5.72-8.35). There’s no significant difference on Initial ART regimens and type of treatment center.Conclusions1. The virological suppression of the late ART group is better than that of the early ART group. Factors that affect viral suppression are transmission routes and types of treatment center, in addition to baseline CD4 cell count.2. The retention rate 12 months following the initiation of the the late ART group is better than that of the early ART group. Factors that affect retention rate are age, marital status, transmission routes and initial treatment regime, in addition to baseline CD4 cell count.3. Till the end of the study, we failed to find there’s any difference among the three groups on mortality. Factors that affect mortality are gender, age and transmission routes.4. The importance of treatment retention and adherence should be emphasized for high CD4+ patients newly initiated to ART therapy through education and counseling programs.
