Objective:To evaluate the clinical value of white blood cell count(WBC)in the diagnosis of neonatal early sepsis(EOS),and to discuss the upper threshold value of WBC.Methods:Three hundred and six cases of neonatal early onset sepsis were selected from January 2019 to March 2020 in our hospital,and 580cases of non-infected group were selected as the control group during the same period.Results:the risk factors of EOS were:twins,amniotic fluid pollution,spontaneous delivery,maternal prenatal fever,gestational age and birth weight(P<0.05).According to the standards of different treatment schemes in the 2003 and 2019 versions,the positive rate of WBC was low(77.6%and 65.1%),but the specificity was high(95.6%and 99.9%).According to ROC curve analysis,the optimal upper limit of WBC was more reasonable in the 2003 edition of the standard vertebrae.Further logistic regression and ROC curve analysis showed that the combination of non-specific indexes was more accurate in the diagnosis of EOS.Conclusion:the negative predictive value of WBC in the diagnosis of EOS is higher than the positive diagnostic value.The clinical application should be combined with the comprehensive analysis of other indicators.The upper threshold value of?25×10~9/L is more reasonable.
