Objective:Investigating the incidence of hypertension, myocardial infraction and/or cerebral apoplexy and the prevalence of risk factors among the residents of Yongfu county and Liujiang county to provide references for local cardiovascular disease prevention. Method:677 residents of Yongfu, Guilin and 870 residents of Liujiang, Liuzhou were recruited by the method of stratified multistage sampling. They were surveyed and their heights, weights, waistline, blood pressure, visceral fat rate, biochemical detection index from blood sample, U-ALB were tested and recorded for comparative studies. Result: ①Altogether 1547 residents were surveyed,870 of them were from Liujiang county. Among them,267 were male(30.7%) with an average age of 56.91±11.31,603 were female(60.3%) with an average age of 56.62±11.41.677 of the participants were from Yongfu with an average age of 60.07±12.20.208 of them were male with an average age of 62.69±11.00,469 were female(69.3%) with an average age of 58.90±12.54 (figure 3,figure 4). Compared with the residents of Yongfu, the residents of Liujiang had a younger average age (60.07±12.20 VS 56.91±11.31;male:62.69±11.00 VS 57.55±11.08;female:58.90±12.54 VS 56.62±11.41,P< 0.01). In addition, among Liujiang residents, those over 65 years old occupied a lower share (39.44% VS 25.17%, P< 0.01) while those with a family history of hypertension had a greater percentage (10.49% VS 16.44%, P<0.01). However, there were no significant differences between the two groups in gender ratios and in the percentage of smokers (P>0.05,figure1, figure 2). ②Compared with the male residents of Yongfu, the male residents of Liujiang had higher heights (1.61±0.57m VS 1.63±0.62m), greater weights (56.52±9.53Kg VS 63.19±9.66Kg), bigger BMI(21.83±3.10Kg/M2 VS 23.68±3.08Kg/M2), greater waistline(76.97±9.71cm VS 84.19±9.24cm), higher visceral fat rate(8.18±4.31 VS 10.48±4.45), greater TC(4.94±0.85mmol/L VS 5.23±1.03mmol/L), LDL-c (2.95±0.72mmol/L VS 3.17±0.84mmol/L), higher blood glucose (5.05±1.22mmol/L VS 5.42±1.08mmol/L), higher blood potassium (4.11±0.35mmol/L VS 4.80±0.44 mmol/L)(P< 0.01,figure3). However, there were no significant differences between the male residents in both areas in TG, HDL-c, Cr and U-ALB (P>0.05,figure3).③Compared with the female residents of Yongfu, the female residents of Liujiang had higher heights (1.49±0.60m VS 1.52±0.62m),greater weights (47.88±8.64Kg VS 55.12±9.47Kg), bigger BMI(21.58±3.31Kg/M2 VS 23.91±3.57Kg/M2), greater waistline(73.97±8.83cm VS 81.80±9.71cm) and higher visceral fat rate(5.12±3.08 VS 7.39±3.67), TC(5.08±0.95mmol/L VS 5.24±0.97mmol/L), LDL-c(3.03±0.81mmol/L VS 3.16±0.84mmol/L), higher blood glucose (4.78±0.89mmol/L VS 5.37±1.26mmol/L), higher blood potassium(4.10±0.36mmol/L VS 4.58±0.41mmol/L)(P< 0.01). But TG, Cr of the female residents of Liujiang were lower than those of the resident of Yongfu(TG:1.46±0.93mmol/L VS 1.33±0.78mmol/L,87.61±20.45μmol/L VS 84.90±19.26μmol/L,P<0.05). There was no significant difference between the female residents in both areas in U-ALB (P>0.05,figure4).④The general incidence of hypertension among Liujiang residents was 35.40%(20.68% after normalized), while incidence rate of hypertension among Yongfu residents was 39.88%(20.26% after normalized). There was no significant difference between the two areas. In both counties, the incidence among male residents was higher than that among female residents (Yongfu:49.04% VS 35.82%, P< 0.05, Liujiang:43.45% VS 31.84%, P< 0.05). However, there was no significant difference between the two areas in incidence among male and female residents respectively, nor was there great difference in incidence in different age groups of the two areas (figure 5). Logistic regression analysis found that incidence of hypertension in Yongfu county was associated with age and waistline; while incidence in Liujiang county was positively correlated with age, waistline, TC, blood glucose and negatively correlated with female gender. ⑤Compared with the residents of Yongfu, the residents of Liujiang had higher hypercholesterolemia incidence, higher low density lipoprotein incidence, higher high density lipoprotein incidence, higher mAlb, obesity or abdominal obesity and higher PAD incidence (P< 0.05). There were no significant differences between the two areas in myocardial infarction incidence and/or cerebral apoplexy incidence, hypertension incidence and diabetes incidence(P>0.05). Logistic regression analysis found that myocardial infarction incidence and cerebral apoplexy incidence among Yongfu residents was associated with age. It was also found that myocardial infarction incidence and cerebral apoplexy incidence among Liujiang residence was associated with hypertension incidence. For residents of Liujiang, those with hypertension had an increased risk of 3.6 of developing myocardial infarction and/or cerebral apoplexy(95%CI:1.234-10.597), while those with diabetes had an increased risk of 1.2 of developing myocardial infarction and/or cerebral apoplexy. ⑥It was found that there was no significant difference between the two areas in diabetes incidence(4.43% VS 5.98%,P>0.05). Diabetes incidence among Yongfu residents was associated with age, while that among Liujiang residents was related to age, waistline, TG and hypertension. For Liujiang residents, those with hypertension had an increased risk of 3 developing diabetes (95%CI: 1.407-6.478,figure 12). Conclusion:The sampling survey on Yongfu and Liujiang residents found that male and female residents over 65 years of age in Yongfu composed a far greater share than those in Liujiang. Liujiang residents had higher heights, body weights, greater waistline and visceral body fat rate, greater obesity incidence and abdominal obesity incidence, higher blood lipid level and higher blood lipid abnormality than Yongfu residents. Also, no significant differences were found between the two areas in hypertension incidence, coronary disease incidence and/or cerebral apoplexy incidence. But hypertension incidence in the two areas was found to have different risk factors. Hypertension among Yongfu residents was associated with age and waistline, while that among Liujiang residents was associated with age, waistline, cholesterol level and blood glucose. Coronary disease and cerebral apoplexy also had different risk factors in the two areas. These diseases were correlated with age in Yongfu, while with blood glucose and hypertension in Liujiang. In Liujiang, residents with diabetes had an increased risk of 1.2 of developing myocardial infarction and cerebral apoplexy, whereas those with hypertension had an increased risk of 1.2 of developing myocardial infarction and/or cerebral apoplexy. Diabetes incidence among Liujiang residents was 5.98% and was 4.43% among Yongfu residents. There was no significance between the two areas. Diabetes incidence in Yongfu was found to be associated with age, while that in Liujiang was found to be correlated with age, waistline, TG and hypertension. Those with hypertension in Liujiang had an increased risk of 3 of developing hypertension.
