Objective: To observe the effects of different surgical approaches on corneal endothelial cells in patients with high mypia and cataract. Methods: 86 patients(86 eyes) with high mypia and cataract from Sep 2014 to Dec 2014 were selected in a prospective, randomized, controlled study, and divided into two groups, group A(46) with phacoemulsification and group B(40) with extracapsular extraction. Record the changes of visual acuity and corneal endothelial cells before and after treatment. Results: 1. There were no statistically different in age, the length of axis and preoperative vision; After treatment the visual acuity was improved significantly in both groups(P <0.05), with no statistical difference between two groups(P>0.05). 2. Repeated measurement anova results: there are no interactions in corneal endothelial cell density between the two groups. There were significantly differences in the density and proportion of corneal endothelial cells(P<0.05), but no statistical difference between two groups(P >0.05). 3. Repeated measurement anova results: there are no interactions in ratios of hexagonal cells between the two groups. There were significantly differences between pre-and post-operative ratios of hexagonal cells in both of groups(P <0.05), but no statistical difference between two groups(P >0.05). Conclusions: 1. Phacoemulsification and extracapsular extraction have a certain extent damage on corneal endothelial cells. However, both two surgical approaches show equivalent efficacy in improving visual acuity of patients with high mypia and cataract. 2. For more ideal operation effect, the technique of phacoemulsification for hard nucleus cataract is small incision cataract extracapsular extraction with chopping nucleus. 3. The ways of cataract surgery should be selected referring to the applicable human populations and their respective advantages and disadvantages, treated correctly, with a reasonable choice.
