Objective:To evaluate the clinical outcome of phacoemulsification by using Balanced energy system and Torsional ultrasonic system in different nuclear density.Methods:A total of 122 eyes(120 cases)of senile cataract with nuclear density grade of III and IV using the EMERY classification were randomly divided into experimental group(using Balanced energy system,59eyes)and control group(using Torsional ultrasonic system,63eyes).Uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),intraocular pressure(IOP)were measured preoperatively,1 day,7 days,1 month and 3 month postoperatively.Corneal endothelial cell density(ECD)was measured preoperatively and 3 month postoperatively.Endothelial cell loss rate was calculated.The main outcome parameters during operation included the cumulative dissipated energy(CDE),case time(CT),aspiration time(AST)and estimated fluid used(EFU)in two groups were recorded and compared.Results:The postoperative UCVA and BCVA in two groups were significantly improved than those preoperatively.The BCVA on the first postoperative day in experimental group were better than control group.In control group,the UCVA and BCVA of ? nuclear density on the first postoperative day were better than those of? nuclear density.In the group of IV nuclear density,the UCVA and BCVA on the first postoperative day in study group were better than control group.The ECD in each group on 3 month postoperative day were significantly lower than those on preoperative day.In the group of IV nuclear density,the Endothelial cell loss rate in experimental group were lower than control group.The CDE,AST,EFU in experimental group were lower than control group.In the group of III nuclear density,the CDE in experimental group were lower than control group.In the group of IV nuclear density,the CDE,AST,EFU in experimental group were lower than control group.Conclusion:Both modes are safe and effective in senile cataract of different nuclear density,however the Balanced energy system showed less US energy and fluid used,caused faster visual recovery in early time and therefore it is advised to use this system in clinical practice.
