【Objective】To study the efficiency and effect on corneal endothelium density of torsional mode for cataract phacoemulsification.【Methods】125eyes of 99 senile cataract patients were prospectively studied and randomly assigned to underwent phacoemulsification either with conventional burst mode or torsional mode in Shandong Provincial Hospital from September,2006 to May,2007. All patients underwent nuclear sclerosis grading and central endothelial cell count (CECC)before operation.Ultrasound Time(UST)and Cumulative Dissipated Energy (CDE)were recorded.Observed corneal clarity at 1 day,and CECC at 90 days after surgery.【Results】For both nuclear sclerosis GradeⅢandⅣcataract,a)the mean UST was significantly lower for the Torsional Group(F=13.52,P<0.01);b)the mean CDE was significantly lower for the Torsional Group(F=18.10,P<0.01);c)the corneal clarity was significantly better in the Torsional group(X~2= 5.85,P<0.05)at 1st day post-operatively,d)the CECC was significantly higher in the Torsional group (F=10.35,P<0.01)90 days after surgery.【Conclusion】:Comparing with conventional ultrasound burst mode,torsional ultrasound mode uses shorter UST,lower CDE and causes less damage to the corneal endothelial cells. Torsional ultrasound mode had more efficiency and effect on corneal endothelium density.
