Objtective: To investigate and compare the effects of modern extracapsular cataract extraction surgery and phacoemulsification on dry eye symptoms, signs and diagnostic test in patients with age-related cataract. Methods :118 elderly patients(128 eyes) with age-related cataract were divided into two groups:Group A received extracapsular cataract extraction combined with intraocular lens(IOL)implantation,50cases(60eyes),and group B received phacoemulsification combined with intraocular lens(IOL) implantation,68 cases(68eyes).The dry eye symptoms, signs,breaking-up time(BUT), the cornea fluorescein staining scores(CFL) and the Schirmer I test of all patients were observed and analyzed at 1 days before surgery and postoperative 7 days, 30 days. Results :7 days after operation, patients in the A group had obvious dryness and gravel sense, SIT, CFL scores were obviously higher than those of the B group, BUT was shorten,the difference was statistically significant(all P <0. 05),.30 days after operation,the difference of each index between two groups was no statistical significance. SIT and BUT has obvious negative correlation before and after operation. Conclusion :ECCE and PHACO both effect on tear film stability in the 7 days after operation, but it can be recovered to the preoperative level in 30 days after operation. ECCE has more effects on ocular surface in 7 days after operation, there are no significant difference after 30 days.BUT and CFL has obvious negative correlation,.The SIT test is not significant in diagnosis and prognosis of operation. The tear film instability is the main reason of dry eye after operation,Preventions and treatment should be taken clinically.
