Objectives:Menstrual headache, as a highly prevalent and financially and mentally costly disease, is posing an increasingly highlighted threat on women’s health, career and daily life. However, insufficiency in knowledge on its pathogenic mechanism has been limiting the therapeutic approaches in both traditional and modern medicine, it urges for a major breakthrough. Therefore, a effective treatment to menstrual headache will be of great significance for clinical practice. Nowadays, most women are in habit of using coolers and cold drinks, so they are vulnerable to blood deficiency and congealing cold induced menstrual headache. Based on this pathogenic mechanism, modified Dang Gui Si Ni decoction is the right cure to blood deficiency and congealing cold induced menstrual headache for its effect of warm the meridian to dissipate cold. In order to solve the inconvenience of boiling the herbal medicine to prepare the decoction, Wen Jing Yang Xue ointment recipe, a modified formula based on Dang Gui Si Ni decoction, is prepared. As it is boiling-free and convenient to carry, additionally its effect is more moderated and prolonged, this preparation is more welcomed by modern women who are living a fast-pace life, and thus patient compliance can be assured. In the present work, a prospective random control clinical trial was conducted to evaluate and compare the efficacy of Wen Jing Yang Xue ointment recipe, aiming at developing a more effective and convenient treatment to menstrual headache.Methods:This research is a prospective random control clinical trial. A total of 38 patients who met the inclusion 7st medicine and placebo capsules were given as control. Usage and dosage of the test group:start at the fifth day of menstrual period,15 mL, q. d for 3 months; the control group:start at the fifth day of menstrual period,2 capsules, q. d for 3 months. Eventually,37 cases were completely investigated with one missed case in the control group (test group:23, control group:14). VAS for headache, headache index and Chinese medicine sign scale were employed as parameters to comprehensively evaluate the efficacy of Wen Jing Yang Xue ointment recipe on blood deficiency and congealing cold induced menstrual headache.Results:1. Response rate:total response rates of the test group and the control group were 87.0% and 21.4%, rank sum test indicated the difference was statistically significantly (P<0.05).2.Response rate based on Chinese medicine disease symptom scale: evaluated based on Chinese medicine disease symptom scale, total response rates of the test group and the control group were 78.3% and 7.1%, rank sum test indicated the difference was statistically significantly (P<0.05).3. VAS scores:VAS score difference of the test group was significant as suggested by rank sum test (P<0.05), whereas the difference was not significant in the control group (P>0.05). The intergroup difference was significant as indicated by rank sum test (P<0.05).4.Headache index:Headache index difference of the test group was significant as suggested by rank sum test (P<0.05), whereas the difference was not significant in the control group (P>0.05). The intergroup difference was significant as indicated by rank sum test (P<0.05).5. Chinese medicine sign scale:Chinese medicine symptom scale score difference of the test group was significant as suggested by rank sum test (P<0.05), whereas the difference was not significant in the control group {P>0.05). The intergroup difference was significant as indicated by rank sum test (P<0.05).6.5-hydroxytryptamine:The intergroup difference was insignificance as indicated by rank sum test (P>0.05).7. Prostaglandin F2 a:The intergroup difference was insignificance as indicated by rank sum test (P> 0.05).8. Prostaglandin E2:The intergroup difference was insignificance as indicated by rank sum test (P> 0.05).Conclusion:Condition of blood deficiency and congealing cold induced menstrual headache can be effectively improved by Wen Jing Yang Xue ointment recipe, as shown by improvements in VAS and headache index. Also, signs of menstrual headache in Chinese medicine were improved. No side effect was observed during the therapy. Wen Jing Yang Xue ointment recipe is an effective treatment to blood deficiency and congealing cold induced menstrual headache.
