Periarthritis of shoulder is a common clinical disease, especially in acupuncture, Tuina and osteological department, which occurs in people over the age of 50. Its symptoms include the pain and limited joint activities of shoulder. It may seriously affect the acts of the patients using their shoulder, such as washing face, combing hair, clothing, etc. It may reduce quality of, life, too. The acupoint of Biheyang is proposed by the family of academician Cheng Shennong, which is basing on nearly a hundred years of clinical experience. And it has also being used to the treatment of periarthritis of shoulder for countless times and achieved good effect. But there is no formal study to report its effect.ObjectiveIn order to propose a more convenient, more effective and more efficient method of clinical treatment of periarthritis of shoulder and provide the theoretical and practical basis of it, we observe the clinical efficacy of the movement therapy on periarthritis of shoulder by pressing the acupoint of Biheyang.MethodsWe selected 60 patients who meet the diagnostic criteria, inclusion criteria and exclusion criteria, and then according to the ratio of 1:1, randomly assigned them to treatment group and control group, each with 30 cases. Then we recorded the gender, age, course of disease and location of the disease. The patients of the control group were treated with traditional shoulder massage therapy, while the treatment group, after the traditional massage therapy, were increased by the movement therapy of pressing the acupoint of Biheyang. Before and after treatment, we measured and recorded the scores of the Visual Analogus Scale, the shoulder joint function scale and the shoulder joint activities range. After that, we used the SPSS19.0 software for statistical analysis.Results①There were both 30 patients of the two groups. It appears that women were slightly more than men; and the incidence of left shoulder was slightly higher than that of the right shoulder. The ages of the two groups were ranged from 46 to70 years old. The average age of the treatment group is 57.8 years, while the control group was 57.53 years. Of the treatment group, the courses of the disease were ranged from 2 to 23 months, while they were from 2 to 21 months of the control group. After using the chi square test or t test, the differences between the two groups of the gender, age, course of disease and location of the disease were not statistically significant (P>0.05). So that the two groups were comparable.②Before treatment, measured and recorded the scores of the VAS, shoulder joint function, activity and the activity range of flexion, abduction, and the distance between the thumb and spinous process of the seventh cervical vertebrae, and the distance between the middle fingertip and ear. After using the t test, the differences between the two groups of the seven aspect above were not statistically significant (P>0.05). So the two groups were comparable.③After treatment, measured and recorded the scores of the VAS, shoulder joint function, activity and the activity range of flexion, abduction, and the distance between the thumb and spinous process of the seventh cervical vertebrae, and the distance between the middle fingertip and ear. After using the t test or Mann-Whitney rank sum test, we found that the differences between the two groups of the VAS score are statistically significant (P=0.035<0.05), while the rest six aspects were not statistically significant (P>0.05). Besides, in addition to the shoulder joint activity score and the distance between the middle fingertip and ear, there were significant difference between the differences between before and after treatment of the rest five aspects (P<0.05). Tips:the treatment of the treatment group in smoothing the pain was better than that of the control group, while in the other aspects two groups had considerable effect.④Comparing the differences between the two groups of patients before and after treatment in the scores of the VAS, shoulder joint function, activity and the activity range of flexion, abduction, and the distance between the thumb and spinous process of the seventh cervical vertebrae, and the distance between the middle fingertip and ear, after using the t test, we found that the differences were significant (P<0.01). Tips:two kinds of treatment can effectively improve the patients with periarthritis of shoulder pain and joint activity situation.⑤After a course of treatment, the total efficiency of treatment group was 96.67%. Among that,7 cases were clinical cured,15 cases were markedly effective, and 11 people were effective. In addition, there were 1 null and void. And in the control group, the total effective rate was 90%, the clinical cured people were 7, and 12 were markedly effective, and 11 people were effective. In addition, there are 3 person is invalid. The total effect of the two groups by Mann-Whitney rank sum test comparison showed that the difference was not statistically significant (P>0.05). Tip:considering the treatment of shoulder joint activity degree and pain, traditional massage can effectively cure periarthritis of shoulder, and based on that, pressing on Biheyang for treatment can increase the efficacy, but we still can not believe that this effect increased significantly.ConclusionTraditional massage for the treatment of periarthritis of shoulder, can effectively improve the seven aspects of the scores of the VAS, shoulder joint function, activity and the activity range of flexion, abduction, and the distance between the thumb and spinous process of the seventh cervical vertebrae, and the distance between the middle fingertip and ear. Based on that, pressing on Biheyang of the treatment group was better than that of the control group in relieving the pain. And the difference between them was significant. While the others had no statistical significance. Therefore, it can be considered that the movement therapy of pressing on Biheyang can relieve periarthritis of shoulder pain significantly, while there is no evidence prove that it can increase the effect of improving shoulder activity.
