ObjectiveThis experiment provided clinical guidance for post-and-core crown restoration by comparing the clinical curative effect of Ni-Co custom cast post, titanium screw post and glass fiber post.MethodFrom Jan 2009 to Feb 2010, a total of 100 patients with large tooth defect were selected from the Career Academy, Zaozhuang, and 150 diseased,.teeth were treated.150 teeth with large area defect were divided into three groups equally. There are no difference between the age and sex of patients in each group. The first group were repaired with Ni-Co custom cast post and PFM crown;the second group were repaired with Titanium screw post and PFM crown;the third group were repaired with glass fiber post and PFM crown.The following five curative effect standard of clinical evaluation were:first, post-and-core is broken;Second, whether the root fold;Third, with or without secondary periapical inflammation;Forth, whether post-and-core fall out;Fifth, with or without gingivitis.The chi-square test were used by SPSS 15.0, p< 0.05 for the difference is statistically significant.ResultsThe results of 150 patients For 5 years of follow-up are:1.There are statistically differences (P<0.05)by comparing the three groups in root fold, post-and-core out and with or without gingivitis. (1) root fold:10 cases happened after the titanium screw post repair,8 cases happened after the custom cast post repair, 1 case happened after the glass fiber post repair. Compered with glass fiber post, titanium screw post and custom cast post’s incidence of root fold is higher. (2) post-and-core out:15 cases happened after the titanium screw post repair,11 cases happened after the custom cast post repair,3 cases happened after the glass fiber post repair.Compered with glass fiber post, titanium screw post and custom cast post’s incidence of post-and-core out is higher. (3) gingivitis:9 cases happened after the titanium screw post repair,8 cases happened after the custom cast post repair,1 case happened after the glass fiber post repair.Compered with glass fiber post, titanium screw post and custom cast post’s incidence of gingivitis is higher. There are no statistically differences(P> 0.05)by comparing the three groups in post-and-core is broken and with or without secondary periapical inflammation. (sheet 1)2. (sheet 4)ConclusionPerformance of finished product of glass fiber post anti root fracture ability, anti shedding performance and fatigue resistance and appearance are better than the Ni Cr alloy casting metal post and refined pure titanium screw pile and finished fiber pile than nickel chromium alloy casting metal post and refined pure titanium screw pile is more conducive to the health of periodontal tissue, nickel chromium alloy casting metal post and refined pure titanium screw pile is easy to cause inflammation of the gingival tissue. And the three had no significant difference in the control of the disease control in the apical tissues. The effect of the fiber post is better for the repair of large tooth defect.
