ObjectiveThe purpose of this study was to evaluate the clinical effect of fiber posts comparing with cast metal posts in a large area of tooth defect.Materials and methodsWe chose 110 patients treated with post crown from the Second Affiliated Hospital of Zhejiang University. The 50 patients were treated with fiber post crown and the other 60 were treated with cast metal post crown. Then evaluate the clinical effect from six affects (crown loss, fracture rate of post-core, root fracture, gingival inflammation, gingival border color, secondary periapical periodontitis) of these two groups after two years follow up. All the cast metal posts in this study were manufactured from Hangzhou Wanmei Dental Co.Ltd. Fiber posts were POPO fiber posts. The resin cements were 3M ESPE self-bonding resin cements. Glass inomer cement was GC Fujiâ… .Statistical analysis was carried out by the SPSS software package(SPSS,15.0 for Windows).ResultThere was significant difference (P<0.05) of crown loss between fiber posts(1) and cast metal posts(11). There was no statistical significance (P> 0.05) of the fracture rate of post-core between fiber posts(1) and cast metal posts(1). There was significant difference (P<0.05) of root fracture between fiber posts(none) and cast metal posts(9). There was significant difference (P<0.05) of gingival inflammation between fiber posts (none) and cast metal posts (8). There was significant difference (P<0.05) of gingival border color between fiber posts (none) and cast metal posts (8). These two groups all didn't have secondary periapical periodontitis.ConclusionThe fiber posts had superior property of bonding, fracture resistance and fatigue resistance than cast metal posts. The fiber posts were better to paradentium than cast metal posts. But these two groups had no significant difference in controlling periapical periodontitis. Therefore, the fiber posts are the safe materials and better than cast metal posts to restore the tooth structure.
