Catheter-associated urinary tract infections (CAUTI) is a series of infections of urinary system which usually occur after placing or removing urinary catheter for the patient within 48 hours. In the intensive care unit, to observe the changes of the patient’s condition, often need to indwelling catheter, accurate monitoring of the patient’s urine volume. And CAUTI is the most common complications of indwelling urethral catheterization, also is one of the most common nosocomial infection in ICU, is one of the important factors in the success of treatment, but also lead to patients with prolonged hospitalization and increased hospital costs.The main measures in the clinical control of CAUTI is to strengthen the aseptic operation, reduce the incidence of infection, although achieved certain results, but still can not reduce the incidence of CAUTI fundamentally CAUTI is often the result of many factors, clinical nursing should be integrated into the overall situation of the patients, taking the targeted preventive measures to reduce the incidence of infection, so the introduction of the concept of cluster of care. Nursing care is a new type of evidence based nursing model, which is a group of nursing intervention measures, which is mainly aimed at a certain type of clinical factors, and their common implementation is better than the individual implementation of the outcome, the current domestic and foreign research mainly focused on the treatment and care of critically ill patients.Objective:Multiple high-risk inducing factors of CAUTI were considered entirely in this study from the perspective of prevention. To select the specific nursing measures, a large number of literature was referenced. And then, the selected unilateral effective nursing measures were integrated into the bundle nursing scheme which conducted in clinical application to study its effect on preventing or reducing the risk of CAUTI. The study aimed to supply the theoretical and practical evidence to CAUTI prevention.Methods:1.The construction of the nursing program of the cluster is established by PICO format, and the related database is established, and the quality of the research literature is evaluated according to the results of the evaluation.2, Clinical intervention in this study, a case control study was selected in September 2013 to 2014 August in a third grade class-A hospital ICU indwelling guide catheter patients as the object of observation, in September 2013 to 2014 February in the ICU hospital 110 cases of patients with indwelling catheter as routine care control group,2014 March to August 2014 in the ICU hospital 110 cases of patients with indwelling catheter as the implementation of the cluster integrated care program in the experimental group, evaluation the cluster comprehensive nursing measures in reducing CAUTI rates in effect.Results:1.The bundle care programs including:good hand hygiene; set a reminder mechanism, early catheter removal; use large capacity anti back flow lash urine collection bags, changed once a week; the use of self-made catheter paste, to catheters were secondary fixation; do a good job in urinary system of terminal care to prevent retrograde infection.2. The incidence of CAUTI was 10.89% in the experimental group, while the control group was 33.66%, and the difference was significant (P<0.05).Conclusion:Based on the rigorous evidence theory, this paper constructs a cluster nursing program of CAUTI, and studies the clinical intervention, it is proved that this program can effectively reduce the incidence of CAUTI.
