Current Status And Strategy Research Of Prevention And Control Of Infectious Diseases Among Primary And Middle Schools And Kindergarten Units In Xihu District Of Hangzhou

Objective To know the current situation of prevention and control of infectious diseases among primary and middle schools and kindergarten units in Xihu district of Hangzhou,and find the week link of infectious diseases prevention and control in schools.So as to provide the decision basis for communicable disease control in schools and to establish the valid mechanism of prevention and control,and put forward the targeted strategy of prevention and control.Methods To use questionnaire method to field investigate the sitiuation of communicable disease control among 105 primary and middle schools and kindergarten units enrolled by the administrative department of education and influence factors of communicable disease control according to the questionnaire design content.The index of communicable disease control in schools and weight of each index were determined and Index system was established by means of the Delphi Method after two rounds of expert consultation.Using EpiData3.0 to entry survey data, using Excel2007 and SPSS19.0 software for data statistics analysis.Results 105 primary and middle schools and kindergarten units had explicit Leaders in charge of the management of infectious diseases.98.1% built infectious diseases management organization.School infectious disease outbreaks emergency plan and registration and reporting system owning rate were both 100.0%,rate of morning check was 99.0%,rate of student absent because of illness etiology investigation and register was 97.1%.99.0% have set up the clinic,25.7% had the practice license of medical institution, health care workers with medical license accounted for 74.3%.The rate of health care workers in primary and middle schools and kindergarten units reaching the standard were 71.9% and 79.2% respectively,health care workers who were in establishment accounted for 82.5% and 37.5% respectively,who participated in system training of infectious disease prevention and control occupied 90.5%.67.6% schools have set up patients or suspected patients of infectious diseases isolation under observation room.98.06% school canteen had the validity period of the hygiene licence,all the canteen had a system of tableware disinfection.Drinking water in school was from purification of drinking water straight or Insulation barrels and electric heating boiled water machine. The qualification rate of bacteriology indicators by water quality sampling was 92.8%,with rate of straight drinking water was 97.6%, the heat insulation barrels water percent was 87.4% and the boiled water machine was 73.6%.Schools were all flush toilets.The rate of decontaminationoffeces was 88.6%.96.20% of toilets set the water washing facilities,81.0% equipped with hand sanitizer sterilized items, etc.classroom and dormitory’s transom window settings percent of pass were 72.4% and 63.6%.This study conducted two rounds of expert consultation.Effective recycling of questionnaire was 100%. Experts’Familiarity coefficient was 0.87,judge coefficient was 0.78,Authority coefficient was 0.83.The index importance of overall coordination coefficient in two rounds of expert consultationwere 0.214 and 0.390 respectively,P values were both less than 0.0001.Finally an evaluation index system of prevention and control of infectious diseases in school with 8 first grade indexes and 32 secondary index was got by filtrating.Thereinto,3 indexes, which were "Schools providing students with drinking water according with drinking water standards","finding suspicious aggregation epidemic and reporting to the education department and the department of CDC report in time, informing the parents of students","After finding students(teachers) suffering from infectious diseases,isolating them by rule",had the highest weights in the whole Index system which were 4.46%,4.34% and 4.29% respectively.Conclusion The current situation of prevention and control of infectious diseases among primary and middle schools and kindergarten units in Xihu district overall was good.But there were obvious gap between clinic and school doctor equipment with national related requirements.Part of schools haven’t set up suspected patients of infectious diseases isolation under observation room,classroom and dormitory’s transom window settings was not conducive to the air circulation,the management of school drinking water hygiene should be further Strengthened especially in the boiled water machine water supply management.Effective measures must be taken to strengthen the school health care power configuration and the prevention and control of hardware facilities,to increase professional guidance and supervision of school infectious diseases control work, to further improve the level of school communicable disease control.The Delphi expert consultation method preliminarily established the evaluation index system and weight of school infectious diseases prevention and control.In this study the degrees of experts’ positiveness,authority and coordination were higher.The index system set up should be rather accurate and reliable.It was able to provide a reference basis for prevention and control capacity evaluation of school infectious diseases.
