Objectives1. To learn the current situation and problems of capability and on disease prevention and control of health-care institution and its staff in rural area of Hubei Province, and proceed to provide objective basis for construction and improvement of infectious disease prevention and control system in rural area.2. To develop training programs for improving the staff ability on infectious diseases prevention and control. On the basis of assessing the training effectiveness, exploring training model fitting for the rural area and proceed to provide the technical support for identifying the training priorities and developing training program.Methods1. Literature review was used to learn the capability and existing problems on infectious diseases prevention and control both at home and abroad from retrospective view.2. The capability on diseases prevention and control of county CDC, township hospital, and village clinics were investigated by using self-made questionnaires. The capability on diseases prevention and control of health worker in rural areas were survey by using questionnaire and semi-structured interviews, and analyse the possible influencing factors using Logistic regression analysis.3. Empirical research method was used to carry on a training programme for 45 public health workers on "improving capability on infectious diseases prevention and control in rural areas", training effect was estimated using questionnaire and semi-structured methods.Results1. The results of investigation showed that human resources of age and sex distributions in county CDC, and township hospital were relatively balanced. The age distribution of village health worker was imbalanced which the age above 50 years takes the biggest part and accounting for 34.97% in the total, and the number of male workers were more significantly than females. Prevention medicine personnel were serious shortage in county CDC, township hospital, and village clinics, which only accounting for 8.45% in public health department of township hospitals.2. County CDC, township hospital and village clinics carried out surveillance and reporting issues. The survey results of laboratory testing capability indicated that county CDC was with sufficient equipment, township hospitals were lack of instrument and village clinics were serious lack of laboratory equipments. Although county CDC had carried out the laboratory test of common infectious disease, the testing personnel were shortage and laboratory equipments were with high vacancy rates. Briefly, the capability of laboratory testing cannot meet the needs of infectious diseases prevention and control in the rural areas.3. The results of the capability of health personnel on infectious diseases prevention and control were showed as following. The eligible rate of self ability evaluation, professional knowledge and laws of infectious diseases were 7.52%,52.85%,65.83% respectively. The health workers in county CDC got higher score than them in township hospital, and the score in township hospital was higher than in village clinics. The results obtained by Logistic analysis showed that factors influencing the capability regarding the professional knowledge of infectious diseases might include: the level of the institution, cultural degree, accumulated training time, work seniority, whether to have participant the response of infectious diseases.4. The results of training needs interview showed that the reasons of "why the health worker do not wish to participant the training" might include:busy work, training contents aren"t interesting, training cost, have not been informed. The reasons of "willing to participant the training program" might include:post demanded, self ability development, establish job communicated network, open views.5. Evaluation the training effect by comparing the obtained score of pre-test and post-test on self ability assessment, professional and law knowledge of infectious diseases, and the results demonstrated below. The training program enhanced the confidence, improved the professional knowledge regarding the disease prevention and control. The statistic results of post-test demonstrated the score of self-assessment, professional knowledge and laws regarding the infectious diseases increases significantly compared with pre-test. Results of satisfaction survey showed that 97.78% trainee feel well about the whole training arrangement,100% trainee feel well on the aspects of the training contents and logistic arrangement,95.55% trainee fell well about the training methods and 88.89% trainee give high evaluation on the teaching skill. In the end,97.78% trainee describe that the training programme have a positive effect on prevention and control, early detection, reporting and response of infectious diseases.Conclusions1. The functions of infectious diseases prevention and control are not clear and the management system is with deficiency of dislocation in the primary health institution in rural areas. In view of the problems, paying much attention to economic effectiveness with less awareness on the management, paying a lot of attentions on medical care and neglecting the diseases prevention. 2. Capability of health personnel are varied and difficult to adapt the current needs for infectious disease prevention and control, the overall quality of primary health personnel need to be further explored.3. The primary health institutions are lack of laboratory testing equipment and the laboratory testing abilities need to be raised in the future.4. Training needs assessment is necessary before the training which ensure the training effect.5. The training programme which is a convenient and efficient way can quickly improve the capabilities of health personnel on infectious disease prevention and control. The training plan and materials are suitable for the health personnel in rural areas.InnovationBased on the background of rural areas in Hubei Province, the study discussed the current situation and the shortage about the infectious disease prevention and control in county CDC, township hospitals and village clinics, proceed to put forward the scientific and reasonable suggestion for the corresponding problems. For the purpose of enhance capability of the health personnel, the study designed training scheme suitable for rural areas, developed reasonable training materials and training models.
